VCUHS Cardiac Surgery ICU, 90 hours

·  Monitored, diagnosed, and managed critically ill patients that have various types of heart surgery, such as heart transplants, coronary artery bypass grafts, aortic aneurysm repairs, heart valve repairs/replacements, or implant of heart devices.

·  Assessed the daily orders, admit patients directly from the ER, OR, or from outside hospitals.

·  Learned the cardiac devices and their function, learned the medications in treating heart failure, predicting patient outcomes, and collaborating with various team members on patients plan of care.

A 4-month rotation in the acute sections of the Emergency department. This clinical provided skills such as central line insertions, arterial line insertions, Suturing, assisting with intubations, running ACLS protocol during cardiac arrests, managing acute CVA events, and other acute disease processes. This clinical rotation allowed me to use critical thinking skills and diagnose, treat, and manage ongoing acute traumas, ACS, CVA's, and endocrine issues. During this rotation I became knowledgeable about medications, IV vaso-active drips, bedside ultrasound, and many more interventions. I had vast exposure to Diabetes, Heart failure, abdominal issues, and Acute Coronary syndromes.

Spring, 2013

VCUHS Emergency Department, 225 hours

I worked with an ACNP this semester that would rotate between the ER and Clinical Decision Unit. I had vast exposure in the fast track unit with patients having non-emergent issues. I worked up primary complaints such as dental pain, back pain, Sexual transmitted disease, Fractures/breaks, Dislocations, Diabetes, Heart failure patients, and extremity pain as well as other primary care disease. Great exposure to writing and prescribing proper medications and antibiotic coverage. I also rotated on several occasions through the Acute Care Side and was able to manage CVA events, SAH events, ACS events, Cardiac arrests, overdoses, trauma patient resuscitation, and many more life-threatening issues.

Skills: suturing, I&D abscess, pelvic exams, placement of central lines, arthrocentesis, digital blocks, assisted with intubations, lumbar puncture, and chest tube placements.

In the CDU, I managed patients with acute and chronic disease that had the ability to decompensate. We observed patients for 24 hours while managing their co-morbidities Patients we typically managed were chest pain, sickle cell crisis, non-acute trauma, abscess requiring IV antibiotics, Diabetes (newly onset, DKA, and HHNK,) sepsis, and acute respiratory events. Skills I learned were the steps for chest pain management such as placing orders so the patient will have their tests done like the MIBI and stress test, managing and diagnosing diabetes, heart failure, and other co-morbidities.

Summer 2013: VCUHS Heart Failure team and Cardiac Surgery ICU 135 hours

·  In this rotation I had the unique experience of managing inpatients and outpatients with the CHF team.

·  We mainly rounded on patients requiring Left Ventricular Devices and Total Artificial Hearts.

·  I was able to gain experience in an outpatient setting as well as directly work with different medical teams.
