LONG-TERM SUPPLIER'S DECLARATION OF ORIGIN
The supplier may issue this declaration on the request of the buyer, who uses it as evidence when he applies for a certificate of origin for the exportation of the goods. It should only be used within the European Community. The text in the frame below, suitably completed in conformity with the footnotes, constitutes the declaration of origin. The footnotes do not have to be reproduced. This declaration is to be placed on the supplier's letter paper.

IMPORTANT ! This declaration is not valid for goods having preferential origin status and which qualify for move- ment certificates EUR.1. For such use the “long-term declaration for products having preferential status” according to Regulation (EEC) 1207/2001 is applicable.
