LEVEL IV INFRACTIONS
- Allowing friends, family members or other employees to ride for the purpose of visiting.General Operations section 2.17
- Misuse or abuse of any MOTA equipment, tools, or facilities.General Operations section 2.18

- Running ahead of schedule due to operator negligence. General Operations section 4.19
- Late departure without a valid reason. General Operations section 4.19
- Running behind schedule due to operator negligence. General Operations section 4.19
- Failure to go directly to the starting point of a run or to return directly to the terminal after a shift without proper authorization. General Operations section 4.19
- Running or stopping off route for other than prescribed reasons. General Operations section 4.20
- Skipping stops or pickups without proper authorization. General Operations sections 4.20, 4.26
- Failure to promptly answerradio when radio is operable.General Operations section 4.29, 7.3

- Failure to observe proper procedures for stopping, picking up, dropping off, assisting, standing, or seating passengers. General Operations section 5.2

- Failure to follow proper procedures for keeping the interior of MOTA vehicles and the dispatch area clean. General Operations section 5.4.1, 5.4.3, 7.4
- Allowing inappropriate articles on the vehicle.General Operations section 5.4.2
- Failure to check for and properly turn in articles left on vehicles. General Operations section 5.4.4

- Incurring a tardy two (2) or more times in any four (4) week period.General Operations section 8.1
- Reporting for a driving shift without having the following items in possession: valid Michigan Commercial Driver License with all proper endorsements, valid Medical Examiner’s Certificate, accurate timepiece, and the MOTA-issued identification badge. General Operations section 8.2
