Location(s): The primary location is Dorothy House Hospice Winsley, as well as visiting patients in the community and other Dorothy House Venues.

Dimensions

Team: The Therapy team is a multi-disciplinary team that comprises of Occupational Therapy, Physiotherapy, Complementary Therapy, Creative Arts and Dietetics.

Patients: To work with inpatients, day patients and outpatients as part of the Therapies team

Sites: Inpatient Unit, Day Patient Service, Community and any other Dorothy House Venues

Job Summary/Main Purpose

· To work as part of the Therapies Team to provide an Occupational Therapy service for the assessment, treatment and on-going management of Dorothy House patients, working both with the patient, their families and carers in liaison with the Multi-Disciplinary Teams to achieve agreed goals.

· Provide cover for current staff holidays, sickness, study/teaching leave or periods of heavy clinical pressure, on the IPU/DPU or in the community.

Principal Duties and Responsibilities

1. Clinical

· To assess patient's Occupational Therapy needs in close liaison with other professionals, carers and families.

· To attend report sessions in DPU and contribute to the multi-disciplinary approach to care.

· To be available as a resource to DPU patients and facilitate appropriate groups sessions.

· To formulate a treatment plan, implement the plan, review and up-date as necessary

· To attend multi-disciplinary meetings for the IPU and support other team members in delivering an Occupational Therapy service to patients on IPU, with follow up in the community if necessary

· To be available for Occupational Therapy community visits for patients on DH caseload under the guidance and supervision of a Senior Therapist

· To maintain accurate clinical records on System 1

2. Management

· To be an active member of the Therapies Team to ensure co-ordination of patient care.

· To liaise closely with other members of the Multi-disciplinary Team and, when appropriate, Community Teams (including Social Care, Health and Housing Associations)

· To work with Team manager in supervising and supporting junior members of staff and volunteers.

3. Educational

· To keep up to date with the current research and practice in Occupational Therapy with specific application to palliative care.

· To participate in appropriate in-house training.

4. Professional

· To work within the Professional Code of Conduct for State Registered Occupational Therapists.

· To participate in clinical supervision and reflective practice.

· To work within the policies and procedures of the Hospice.

· To participate in annual Performance Reviews.

· To take responsibility for own personal development, identifying training needs with line manager.

No Smoking Policy

Dorothy House operates a No Smoking Policy for all staff, volunteers and visitors in relation to promoting health. It applies to the Hospice premises and grounds at Winsley, all Dorothy House shops and when staff are on duty in patients’ homes.

Essential / Desirable
· Degree in Occupational Therapy / ü
· Current Registration with HCPC / ü
· Post Graduate Training in Palliative Care / ü
· Minimum of 2 years post graduate clinical experience / ü
· Effective organisational and time management skills / ü
· Experience of working in partnership across organisational boundaries / ü
· Demonstrates a knowledge of evidence based practice and its application in Occupational Therapy / ü
· Experience of working with people in Palliative Care / ü
