Terms of Reference: Service Delivery Model Workstream

Context

The Southern Health and Social Care Trust is currently reviewing day care services for adults with a disability.

The aim of this review is to design and implement a Modernised Disability Day Service which is fit for purpose into the future.

A Project Structure has been established to take forward the Review

Purpose of Group

To review current models of Service Delivery and make recommendations regarding future Service Delivery in accordance with revised Eligibility Criteria for service provision.

Responsibilities and Activities of the Group

This group will take forward the following tasks and responsibilities:

  • Review current Service Models for Day care, Day opportunities, including Forensic Day opportunities, including Independent Sector provision
  • Identify what is working well and what needs to improve
  • Ensure commissioning direction is translated into service provision
  • Ensure Commissioning Teams provide necessary information regarding prevalence, age profile, etc. to inform future service delivery
  • Review interfaces with all current services including completion of assessment of need, reviews and requirement for new pathways
  • Consider feedback from all Workstreams and make recommendations based on findings to inform future service delivery models.

Frequency and conduct of meetings

  • Monthly
  • Duration - max 2hours
  • Venues - accessible
  • Phones to be turned off for duration of meetings
  • Minutes to be rotated amongst members in format of discussion points - task, responsibility for completion and timeframe
  • Minutes to be disseminated to colleagues by group members
