Welcome to the Village at Squaw Valley USA Rooming List Instructions The information requested on the rooming list form is necessary for the Village at Squaw Valley USA to make complete and accurate reservations for your group. Your Conference Planning Manager will be happy to send you an electronic template upon request. Your own list is also acceptable if the same information, as outlined below, is provided. Please also follow the guidelines below when making changes to your rooming list. Room Assignment The Village at Squaw Valley USA will honor the room size requested. We will note your requested building and beddings assignments, but cannot guarantee specific locations, or room numbers. Splitting Rooms Charges If your group attendees will pay for their own rooms, tax and incidentals, and are sharing condominiums, the following must be considered: The Village at Squaw Valley USA is unable to divide certain charges (telephone calls, incidentals, etc). It is the responsibility of the individual whose name appears on the reservation City Ledger to finalize all charges amongst occupants BEFORE checkout. It is the responsibility of the group leader or meeting planner to inform all attendees of the above. Requested Rooming List Information Names Please type or print the last name and then first name of each attendee with title, if any. Under the Main Name on the reservation, please indicate if there are other attendees sharing this room and if so list names as instructed above. Note- the Main Name on the reservation is the name linked to the billing City Ledger. Room Type- denotes the accommodation requested One Bedroom = 1bed/ 1bath- King bedded room with Queen size pullout sofa (sleeps 1-4 people) Two Bedroom = 2 bed/ 2bath- One King size bed, one Queen size bed and one Queen sofa pullout (sleeps 2-6) Two Bedroom with Twins = 2 bed/ 2bath with twin beds- One King size bed, two Twin beds and one Queen sofa pullout (sleeps 3-6) Three Bedroom = 3 bed/ 3 bath- One King size bed, one Queen size bed in the 2nd and 3rd bedrooms and one Queen size pullout sofa (sleeps 3-8) Billing Please indicate next to the Main Name on the reservation whether room, tax and incidentals go to the Master Group City Ledger or to the individual’s City Ledger. If the group wishes to pay for room and tax, but not incidentals, please write Maser/incidentals to “I” Dates Arrival and Departure dates Special Requests Indicate special guest needs VIP notations Handicap access Views- more expensive Room location request Requests are filled on space available basis Changes to Rooming Lists Please list all changes by the date the change was requested on the bottom of the original rooming list. Names If there is a name change, please write down the new name and then note if the name is a new reservation or if the name is replacing an attendee, already on the rooming list. If the new name is replacing an attendee on the list, state which reservation number the attendee being replaced is under and whether the attendee being replaced is moving into a different room or is now not coming. For example: Karen Mack is a new attendee and she will need a 1 bedroom on 8/6/04 and she departs 8/9/04. Room and Tax to the Master City Ledger and incidentals to I. Karen Mack is a new attendee, replacing Tim Nelson who is a share on reservation #10 on the rooming list. Tim Nelson is replacing Kelly James as a share for reservation #6 and Kelly is no longer attending. Dates If there is a date change, please list the name of the reservation and the new arrival or departure date. Billing If there is a billing change, please note the name or the reservation and the new billing quest. If the reservation is being paid in part by the group and in part by the individual please specify which dates are to be charged to who. For Example: o Karen Mack will be arriving on 1/1 and departing on 1/10. Karen will be paying for the nights of 1/1, 1/2, and 1/9, all other nights to the master bill. Special Requests If a special request is no longer needed, or a new special request is added please note the name on the reservation and what the new request is and what is no longer requested. Room Type If there is a change in room type, please note the name on the reservation and the new room type. Classroom Style Most desirable for long lectures that involve note-taking. Banquet Style or “Rounds” Generally used for meals and small group discussions. 72” Rounds seat up to 10 people Hollow Square Style Used to promote open discussion between the meeting participants.
