Women's Centre 7 RainCity LGBTQ2S* $857,349 Housing first project located in Vancouver that will Housing & Youth Housing provide housing and supports to chronic/episodic Support First Program homeless youth who identify as Lesbian, Gay, Bisexual, Society Trans, Queer and Two Spirit. 8 Stepping Stone Langley $332,609 Housing first case manager to work with homeless Community Outreach - outreach team to provide services for clients who are Services Housing First identified as episodically and chronically homeless, using a housing first approach. Stepping Stone established a Housing First Advisory Committee of community partners. 9 Aunt Leah's The Link $532,209 The Link Housing First began operation in January 2015 in Independent Housing First partnership with HPS and BC Housing and serves the Life Skills needs of homeless foster youth who have, or will soon, Society ‘age out’ of provincial foster care system. This housing first initiative focuses on youth from foster care who are experiencing chronic and episodic homelessness. Program brings a preventative aspect to housing first model, to prevent aging out into a street and shelter homelessness. 10 RainCity Housing First $168,970 ICM team for region, with HPS funding case manager and Housing & ICM Team for housing specialist Support Maple Ridge Society 11 RainCity RainCity Surrey $1,238,926 Serve clients and provide scattered site housing to people Housing & ICM Team who are chronic/episodic or those with a history of Support chronic/episodic homelessness in Surrey/White Rock as Society primary location and Langley as secondary location. Place 15 clients with complex addiction issues into permanent housing. 12 The Elizabeth Surrey Housing $3,046,462 A collaborative between E Fry, Options, Lookout and Fry Society of First Sources in Surrey serving chronically and episodically Greater Collaboration - homeless or history of either; goal to place 157 clients. Vancouver Hitting The Nail on the Head 13 Pacific Broadway $460,958 Partnering with the Broadway Youth Resource Centre Community Youth Resource Collective, will house 30 youth aged 0-24 and include Resources Centre Housing Aboriginal community, youth transitioning to adulthood Society Outreach and LGBTQ2S community. 14 RainCity Coquitlam $229,456 To provide scattered site Housing First case management Housing & Housing First to up to 15 people. Case manager will provide Housing Support Case First supports to chronic and/or episodic homelessness Society Management and who have significant and complex issues with a focus on people 55+. 15 The John From Prison to $604,780 Provide wraparound services and connect individuals to Howard Community - permanent housing for successful reintegration into the Society of the Providing community. SUPPORT SERVICES – PRIORITY: to improve the self-sufficiency of homeless individuals and families and those at imminent risk of homelessness through individualized services 16 Circle of Eagles Housing and $279,009 Provide housing and support services; goal to serve 26 Lodge Society supports clients; 13 housing placement and 13 loss prevention. 17 Covenant Right of $363,852 Supportive transitional housing program, providing 25 House Passage bachelor suites to youth ages 16-24 who are homeless or Vancouver at imminent risk of homelessness; provides youth with a structured 6 step life skills program, support with individualized goals and plans, access to educational and employment resources, housing placement, and mental health and addictions counselling. 18 Sources Essentials to $549,762 Support project serving the general homeless populations Community Housing of Surrey, White Rock, and Langley; place clients into Resources stable housing, while their main focus will be in housing Society loss prevention. 19 Cwenengitel Indigenous $232,739 Serve Aboriginal clients between ages of 16 and 65 in Aboriginal Services for Surrey. While Cwenengitel currently has a residential Society Indigenous program serving 14 clients, HPS funding will allow them to People in extend their services to non-residential clients and Surrey provide cultural activities, housing placement, and counselling. 20 Pacific Surrey Youth $239,457 Fund staff wages to provide direct client support to youth Community Housing and and young adults up to age of 24 who are homeless or at Resources Homelessness imminent risk of homelessness in the Surrey/White Rock Society Prevention area; place 125 clients into more stable housing and assist Program 125 clients with loss prevention services. 21 Vancouver Aboriginal $483,448 House 60 clients per year; work with Aboriginal Aboriginal Housing individuals who are homeless or at imminent risk of Transformative Support homelessness with a culturally based approach of Housing Justice Program Circles.
