Speak up if you have questions or concerns about your care. • Tell a member of your health care team about your past illnesses and your current health condition. • Tell your health team if you have any food or medication allergies. • Make sure you know what to do when you leave the hospital. •••••••••••••••••••••••••••••••••••••••••••••••••••••••••••••••••••••••• Your Interprofessional Plan of Care • Your hospital admission for your total hip replacement will follow an “interprofessional plan of care” more commonly called a "Clinical Pathway". • This is your version of the Clinical Pathway. It is an interactive document. Please take notes if necessary when the nurse is discussing with you. • It has been prepared so that you, your family, and the members of the healthcare team know your plan of care. It gives you an idea of the tests, treatments and diet you will require during your hospital stay as well as the education and planning necessary for discharge • Once you and/or your family have read the information in this booklet, please feel free to ask any member of the healthcare team any questions. 4 Table of content 1.
