THE HAYNER PUBLIC LIBRARY DISTRICT OPERATIONS POLICY CIRCULATION POLICIES CHECKOUT PERIODS FOR MATERIALS BY FORMAT The initial checkout period for all Hayner Library materials (except DVDs and software) is two weeks. The initial checkout period for all Hayner Library DVDs and software is one week. Items checked out via Interlibrary Loan are subject to the loan periods of the owning library. CHECKOUT LIMITS FOR MATERIALS BY FORMAT Books: No limit; do not take everything we have on one subject Audiocassettes/Audio CDs/Music CDs: Any combination of 10 per card Videocassettes/DVDs: Any combination of 5 per card Magazines: 5 issues per card Software on Compact Disc (CD-ROM), Leap Pad cartridges, puppets, kits, games, etc.: 5 per card REGULAR RENEWALS The regular loan period for all Hayner Library materials is two weeks (except DVDs and software). All Hayner Library items (except DVDs and software) may be renewed at the borrower’s request as long as there are no outstanding holds on the items. The renewal period is two weeks. The same item cannot be charged to the same borrower more than three times in a row—i.e., a maximum of three consecutive charges, or a total of six weeks. In other words, the same borrower cannot renew the item more than twice. Hayner Library DVDs and software may be checked out for one week. Hayner Library DVDs and software may be renewed one time. After returning these items, they must sit on the shelf for twenty-four hours before they can be checked out again. Items checked out via Interlibrary Loan are subject to the renewal periods (if any) of the owning library. VACATION LOANS—RENEWALS For the convenience of library users, we will charge Hayner Library items (excluding DVDs, software, and materials designated as “new”) on a six-week vacation loan at the request of the library user. Items charged on a six-week vacation loan cannot be renewed. Items belonging to other libraries may not be checked out for longer than the owning library’s initial loan period. However, these materials may be renewed, per the owning library’s renewal policy, as long as there are no outstanding holds on the items. 1 TEACHER LOANS—RENEWALS Bulk loans or teacher book boxes may be charged to teachers on a six-week loan for classroom use. Such bulk loans generally cannot be renewed beyond the initial six-week loan. We will make an exception to this rule under the following circumstances: If a book box is returned and a few items from the book box are missing, we will renew the missing items at the teacher's request for an additional two weeks. We do this as a service for teachers, to allow them to search for and recover the missing items without incurring overdue fines. Such items cannot be renewed a second time. If the items are still missing at the end of the renewal period, the teacher will be expected to pay for the cost of the lost item(s) plus service charge(s). No other materials may be borrowed until the lost items have been paid and cleared from the borrower record. DVDs and computer software cannot be checked out for six weeks. SHARE HOLDS—PATRON NOTIFICATION When items to fill borrower holds are checked in at the agency from which they were placed, the Polaris system automatically begins a countdown of the number of days the agency can hold the items for patron pick-up before the hold is canceled. The Polaris system allows a patron SIX DAYS to pick up a hold. In order to check out an item on hold, a patron will need the library card that was used to request the item. The Polaris system will not allow library staff to check out a requested item to any other borrower ID number except the one that was used to place the request. HAYNER PUBLIC LIBRARY DISTRICT Application form for library card All applicants are required to fill out a library card application form. Adult applicants are required to show proper ID to verify residency in The Hayner Public Library District in order to obtain a library card. Juvenile applicants are required to have a parent or guardian signature on their application card (signed in the presence of a library staff member); the parent/guardian is required to show proper ID to verify residency in The Hayner Public Library District. If a parent/guardian wishes to obtain a library card for his/her child (under age 18), the parent’s library record (and all other library records for minors for which the parent is listed as parent/guardian) must be free of outstanding fines/fees and overdue materials. The child must be present in order for a library card to be issued in his/her name. The child does not need to be present for the library card to be renewed. Applicants must show a street address within the Library District. Post Office Box numbers are not acceptable addresses. Post Office Box numbers may be entered into patrons’ records as the primary address if that is where they wish to receive their mail, but the street address must be listed in their library record. The following identification is required: 1. Photo ID with current address (a current valid driver’s license or state identification). 2. Second proof of residency if the above listed photo ID does not have the patron’s current address. 3 Some acceptable forms of proof of residency are (but not limited to): Utility Bills* o Water and Sewer Bill* o Electric and Gas Bill * o Trash Bill* o Home Phone Bill* o Cable Bill* Property Tax Bill Lease/Rental Agreement (not receipt) Mortgage Payment Occupancy Permit Public Aid Mediplan Card Cellular Phone Bill* Credit Card Bill* * Indicates that the item is required to have been processed and mailed via the U.S. Postal Service. Checks are NOT acceptable forms of ID. There is no minimum age limit on library cards. Parents or legal guardians determine when a child is ready to apply for his/her own library card. In the event the minor becomes of age, it is the parent’s responsibility to have the child obtain individual library status, or the parent will be responsible for any fines incurred on the child’s card. DUPLICATE JUVENILE RECORDS In accordance with a decision by The Illinois Heartland Library System, The Hayner Public Library District will issue duplicate juvenile records for juveniles living in dual households (example: divorced parents). The juvenile may have two records within any two libraries in the Illinois Heartland Library System. If one or both parents reside within the boundaries of The Hayner Public Library District they may each apply for a library card for their child/children. Duplicate library cards are independent of each other; fines, fees, and overdue materials on one card will not affect borrowing privileges of the other. Each parent is responsible for all fines and/or fees accrued on their respective card. Parents are also responsible for all materials checked out on the library card for which they signed. Parents must show proper proof of residency within The Hayner Public Library District when applying for a card for their child. The parent’s library record (if one exists) must be free of fines and/or fees over $5.00 before a library card may be issued in their child’s name. PATRONS WITH DELINQUENT RECORDS Patrons with outstanding fines or overdue items may not check out materials until all overdue items are returned or renewed, and all fines over $5.00 are paid. LOST/REPLACEMENT CARDS: Patrons are charged a $1.00 replacement fee for lost cards. Cardholders must fill out a new application form and present an acceptable form of ID when 4 applying for a replacement card. Juveniles who lose their cards must fill out a new application form with parent or guardian’s signature (signed in the presence of a library staff member). The parent or guardian must show an acceptable form of ID when his/her child receives a replacement card. All outstanding fines and fees over $5.00 must be paid before the borrower’s record is renewed or replaced. CHANGE OF PERSONAL INFORMATION: If a Hayner cardholder has a name change, a change of address within the district, or a change of phone number, he/she should report the change to the library as soon as possible.
