Parking charges apply in Braunton. On-street parking is available in Torrington. The Terrace Café at North Devon Hospice sells hot and cold meals to suit many dietary requirements. You are also welcome to bring your own lunch. Tea, coffee and biscuits will be provided, small donations to the Hospice are welcomed as contributions to cover these costs. Personal requirements: please advise us in advance if you have access or learning support needs. Evaluations: All delegates are asked to complete an evaluation form which allows us to continually review and improve our education and training sessions. Certificates: Certificates of attendance will be issued after the course. An additional Certificate of Recognition will be sent to those who attend all of the outreach sessions. Attendees will learn strategies to overcome barriers and facilitate discussion. Skills learned in this session empower the healthcare professional to keep the individual at the centre of their care and is relevant to all settings. Anticipatory loss, bereavement and grief This session will enable the attendee to recognise physical and emotional symptoms of anticipatory loss, bereavement and grief. We will explore simple yet effective approaches to assess the potential impact of loss on an individual. Attendees will learn strategies to overcome barriers and facilitate discussion. Skills learned in this session empower the healthcare professional to keep the individual at the centre of their care, as well as supporting themselves and each other and is relevant to all settings. Working with the Mental Capacity Act “The Mental Capacity Act 2005 provides a statutory framework for people who lack capacity to make decision for themselves, or who have capacity and want to make preparations for a time when they may lack capacity in the future. It sets out who can take decisions, in which situations and how they should go about this” (Department of Constitutional Affairs, 2007). This session will enable the attendee to understand, recognise and know how to use the Mental Capacity Code of Practice. Skills learned in this session empower the healthcare professional to keep the individual at the centre of their practice, fulfilling their duty of care and is relevant to all settings. Supporting individuals with their Advance Care Planning This session will enable the attendee to understand the principles and practices for planning end of life care. Attendees will learn strategies which will encourage and empower open discussion enabling the person to identify and document their wishes and preferences. We will explore the key documents including Advance Care Plans, Advance Decision to Refuse Treatment and the Treatment Escalation Plan. Skills learned in this session support the person to develop their own end of life care plan and are therefore relevant to all settings.
