Our courthouse estimator, Tom Leonard brings comprehensive knowledge from estimating more than $358 million of correctional projects and a history of highly accurate estimates from earliest possible stages. SECTION 1 Proposal Transmittal Letter PROPOSAL TRANSMITTAL LETTER This Section shall include the completed Proposal Transmittal Letter. The Proposal Transmittal Letter (See Proposal Form 1, appended to this RFP) and all attachments thereto must be signed by a representative of the Proposer who is empowered to sign it and to commit the Proposer to the obligations contained in the Proposal. A completed Certificate of Authorization, which is included as Attachment 1 to the Proposal Transmittal Letter, must also be submitted in this Section. If the Proposer is a partnership, the Proposal shall be signed by one or more of the general partners. If the Proposer is a corporation, an authorized officer shall sign his or her name and indicate his or her title beneath the full corporate name. Anyone signing the Proposal as an agent must file with it legal evidence of his or her authority to execute such Proposal. Proposers shall attach a sample insurance certificate and a letter from the surety named in the Proposal Transmittal Letter, confirming Proposer’s bonding capacity. A complete list of key personnel indicating name of individual and their role shall be provided in Attachment 2 to the Proposal Transmittal Letter and be submitted in this Section. In submitting the identity of any such individuals, the Proposer commits to the Owner that such individuals will remain on the Project in accordance with the requirements of the Agreement. Proposers shall attach a copy of their hourly rates schedule. In Attachment 3 to the Proposal Transmittal Letter, Proposers shall include a copy of any Ohio licenses and certificates of registration held by the Proposer and the individuals named in Attachment 2 which are related to the Work to be performed on the Project, including any architectural and engineering licenses of the professional engineer(s) and or architect(s) that will sign the design drawings and/or documents must be given responsible charge for the design work and shall be licensed in the State of Ohio. Provide proof of current registration for all such professional engineer(s) and/or architect(s).Proposers shall submit a properly completed Delinquent Personal Property Tax Affidavit in the form included in Attachment 4 to the Proposal Transmittal Letter.
