<p>Employee Exit Checklist for Supervisors</p><p>☐ Complete OHR Personnel form and return to Human Resources </p><p>☐ Ensure employee completes and submits final E-Timesheet </p><p>Collect the following items from employee and return to applicable departments:</p><p> Building/Office Keys/Key Fobs o  Employee ID Card  Electronic Devices ☐ o Cell Phone o Ipad o Laptop  Parking Permit (employee should check with Campus Security directly if they are entitled to a refund) o  Purchasing Card  Any other state owned property</p><p>☐ Collect any additional department property, as required</p><p>Determine date to vacate office with employee, if different than last day of employment ☐ Date: ______</p><p>Remind employee to:</p><p> Complete Exit Interview Questionnaire ☐  Complete any remaining expense reports and/or final reconciliation of purchasing card  Redirect any personal mail, email or subscriptions to another address  Pay any outstanding parking fees, if applicable  Return any library materials to Livingston Lord Library</p><p>Employee Name:</p><p>Supervisor Signature: ______Date:______</p><p>Return to Human Resources Department</p>
