<p> PHOENIXVILLE COMMUNITY HEALTH FOUNDATION</p><p>CAPACITY BUILDING MINI-GRANT PROGRAM</p><p> An ongoing mini-grant program for non-profit organizations serving the greater Phoenixville region  Intended to assist non-profit organizations with training and educational programs that strengthen the governance structure, program accountability, and program sustainability.</p><p>PROGRAMS THAT MAY QUALIFY FOR A GRANT INCLUDE:</p><p> Board of Director Training  Strategic Planning  Standards for Excellence Activities  Outcome-Based Program Evaluation  Staff Development Training  Fund Development Training/Resources </p><p>TO APPLY: Complete a Mini-Grant Request Form signed by the current Board Chair. This application is also available on-line at www.pchf1.org</p><p>PHOENIXVILLE COMMUNITY HEALTH FOUNDATION</p><p>Revised 4/8/2018 CAPACITY BUILDING MINI-GRANT PROGRAM</p><p>Background </p><p>The Phoenixville Community Health Foundation has established this mini-grant program to promote good governance and best practices within the non-profit organizations that serve the greater Phoenixville region. This Capacity Building Mini-Grant Program will consider requests for up to $1,000 for capacity-building activities such as strategic planning, board training, outcome-based program evaluations, organizational sustainability, etc.</p><p>Eligibility</p><p> Funding through this Min-Grant program is available one-time within a three- year period  Mini-grants can be requested at any time.  $1,000 is the maximum mini-grant or 50% of the cost of the capacity building activity – whichever is lower.
