We are currently working on a contract and the required elements for the State. I would like to schedule 2 events with you.</p><p>#1. I would like to schedule a mock survey next week with our Compliance Director Susie Wilson if possible. Probably need 2-3 hrs. of your time. November 6, at 12:30 pm-3:30pm Once this is completed and the State has all the required documents, they will send a surveyor to ensure we are in compliance with the alternative care setting regulations.</p><p>#2. We are required to provide education to all MNMOP colleagues regarding the PACE model of care. My suggestion would be to have you attend the full day orientation we are running on 11/30 from 8 – 3 at our Broad Street office and then we would have one of our staff come to your center and provide a shorter version using packets. This is how we currently orient our nursing instructors and then their students. MERCY LIFE PARTICIPATION CRITERIA CHECKLIST: Listed below are basic screening criteria for entry into the LIFE PACE Program. Your participants most likely meet the criteria listed below in order to participate in your Adult Social Day Care Waiver program.</p><p> Participant must live in one of the following zip code areas: 19106, 19107, 19122, 19123, 19125, 19133, 19134, 19137, 19140, 19145, 19146, 19147 or 19148.  Is the participant 55 years of age or older?  Has the participant been approved by PCA as NFCE Eligible?  Does the Participant live in an apartment and/or home. (Excludes boarding homes/shelters).  Verify that the participant does not travel outside of their home zip code for more than 30 consecutive days.  Verify participant’s gross monthly income is at or below $2163/month & they do not have more than $8000 in savings or liquid assets combined.  Verify participant does not own a home, mobile home or property.  Verify participant is currently covered under Medicare, Medicaid or Dual Coverage.  Does the participant have a Life Insurance Policy? If yes, what is the cash value of the policy?  Does the participant have a Primary Care Physician and/or a Specialty Care Physician?  Does the participant receive services from any agency? (Ex. Home care, Personal Care Aid etc.)  Has the participant ever been admitted to a nursing home?</p><p>Segal Arts: Art therapy each Monday Alzheimer’s Memories in the Making Watercolor program each Friday</p><p>Adult Ed Program</p><p>Adult Learners: GED – 30 ABE – 12</p><p>Total: 42 Active Participants </p><p>Teachers – 17 ….. ( 7) Teachers meet with a learner – one-on-one</p><p>10 teachers meet with 2-4 learners in a class setting. </p><p>Computer Class - A 6 week class began in October ….Basic Computer Skills</p><p>Total Enrollment: 8 learners</p><p>Cost: $20 – 6 week course</p><p>Programs sponsored by the Mayor’s Commission on Literacy in which MNM is involved: My Place 5 learners</p>
