This form is to be prepared and submitted by the Consultant at the end of each month. Copies are to be retained by both the Consultant and the State Resident Engineer for each separate job file.</p><p>2. When vehicles are used only for state work, payment will be made at the maximum monthly rate specified in the contract agreement. When a vehicle is used on more than one project in a month, the monthly maximum rate will be prorated to the various projects based on the number of days used on each project. (See Vehicle Nos. 59, 66 & 70 in example below.)</p><p>3. When vehicles are used only for state work, payment will be made at the daily rate specified in the contract agreement up to the maximum monthly rate.
