<p> Disability Day Services Review </p><p>Terms of Reference: Service Delivery Model Workstream </p><p>Context </p><p>The Southern Health and Social Care Trust is currently reviewing day care services for adults with a disability. </p><p>The aim of this review is to design and implement a Modernised Disability Day Service which is fit for purpose into the future. </p><p>A Project Structure has been established to take forward the Review</p><p>Project Steering Group</p><p>Project Team</p><p>Project Sub Groups</p><p>Assessment of Need/ Eligibility Criteria</p><p>Accommodation</p><p>Service Delivery Model </p><p>Human Resources/Staff Engagement and Support Services</p><p>Purpose of Group </p><p>To review current models of Service Delivery and make recommendations regarding future Service Delivery in accordance with revised Eligibility Criteria for service provision.</p><p>1 Membership </p><p> Bronagh McKeown  Noreen McComiskey  Tony Black  Janet McConville  Donna Curley  Melvyn Purdy  Darren Campbell  Lisa Polland-O’Hare  John McNeill  Andrew Hayward  Bernie McNaughton  Marita Higgins  Sandra Wylie  Oonagh Nugent  Leonie Hudson  Sinead Campbell</p><p>Responsibilities and Activities of the Group </p><p>This group will take forward the following tasks and responsibilities:  Review current Service Models for Day care, Day opportunities, including Forensic Day opportunities, including Independent Sector provision  Identify what is working well and what needs to improve  Ensure commissioning direction is translated into service provision  Ensure Commissioning Teams provide necessary information regarding prevalence, age profile, etc. to inform future service delivery  Review interfaces with all current services including completion of assessment of need, reviews and requirement for new pathways  Consider feedback from all Workstreams and make recommendations based on findings to inform future service delivery models.</p><p>Frequency and conduct of meetings</p><p> Monthly  Duration - max 2hours  Venues - accessible  Phones to be turned off for duration of meetings  Minutes to be rotated amongst members in format of discussion points - task, responsibility for completion and timeframe  Minutes to be disseminated to colleagues by group members</p><p>2</p>
