Information from this datasheet should be transferred to the EQuIS data entry template for submittal to the Minnesota Pollution Control Agency (MPCA) by November 1st of each monitoring year. Local partners may retain this datasheet for their records at their discretion. This data sheet should not be submitted directly to the MPCA.
