<p>1/1/2012 OSIS (Onsite Septic Inspection & Maintenance) Maintenance Agreement for HOOT BNR* Units Installed in Maryland</p><p>Basic Maintenance Agreement** Includes one visit/year, answer any alarm calls at no additional charge. Parts out of warranty will be billed to customer at standard rates. Premium Maintenance Agreement** Includes TWO visits/year, answer any alarm calls at no additional charge. Parts out of warranty will be billed to customer at 20% discount off standard rates. Complete Maintenance** Agreement (Min 3 year agreement) Includes two visits/year, answer any alarm calls at no additional charge. Parts out of warranty will be replaced at no cost to customer. </p><p>**Pumping of tanks is not included in maintenance. When the tank needs to be pumped, it will be noted on maintenance form. It will be homeowner’s responsibility to contract for pumping. We can provide list of Hoot trained pumpers.</p><p>Pricing: (Payment of multi-year contract assures you of a fixed price and offers a discount) Contract will transfer if property is sold.
