Maintenance Agreement for HOOT BNR* Units

1/1/2012 OSIS (Onsite Septic Inspection & Maintenance) Maintenance Agreement for HOOT BNR* Units Installed in Maryland

Basic Maintenance Agreement** Includes one visit/year, answer any alarm calls at no additional charge. Parts out of warranty will be billed to customer at standard rates. Premium Maintenance Agreement** Includes TWO visits/year, answer any alarm calls at no additional charge. Parts out of warranty will be billed to customer at 20% discount off standard rates. Complete Maintenance** Agreement (Min 3 year agreement) Includes two visits/year, answer any alarm calls at no additional charge. Parts out of warranty will be replaced at no cost to customer.

**Pumping of tanks is not included in maintenance. When the tank needs to be pumped, it will be noted on maintenance form. It will be homeowner’s responsibility to contract for pumping. We can provide list of Hoot trained pumpers.

If you do not have a maintenance agreement and you need service, you will be billed a service/travel charge of $150 plus $75/hr for time on the job plus any needed parts.

*Pricing is for standard BNR Units HOOT ANR Units require the addition of “Hoot Juice” which will be an added cost of $50 Per year Units with Hoot Drip Disposal, please add $50 per year for maintenance contract add on.

Service Contracts start the day payment is received. If you have an alarm and are out of contract, you must pay a service/travel call fee of $150 plus any additional charges. The purchase of a new service contract would start after the service. 1/1/2012
