<p> COE Installation Quality Bulletin</p><p>Installation Quality Bulletin Number: 05-02 Issue 4</p><p>To: All Contracted Installation Service Suppliers / CenturyLink Quality Managers / CenturyLink Procurement</p><p>Original Issue Date: Friday, April 15, 2005 Reissue Date: Thursday, October 20, 2011 From: CenturyLink Installation Quality Assurance</p><p>Subject: Job Start & Completion Notification Request for CenturyLink-Approved Installation Service Suppliers</p><p>Action Required: YES:  NO:</p><p>Description of Bulletin:</p><p>The purpose of this Quality Bulletin is to inform all CenturyLink-approved installation Service Suppliers that effective immediately their employees and/or representatives that perform COE installation-related work on behalf of CenturyLink, will notify the area CenturyLink Quality Manager(s) (refer to Table A) a minimum of one week before both their proposed Job Start and Job Completion dates. </p><p>The notification process shall consist of an electronic mail transmission along with a courtesy phone call (or voicemail) made directly to each Quality Manager. In the notification, the Service Supplier should refer to the assigned Billing Verififcation And Processing for Payment (BVAPP) order number, central office Common Language Location Identifier (CLLI) code, a brief description of the nature of the work and the proposed Start and Completion dates. </p><p>It’s CenturyLink’s policy to audit 100% of all contracted installation work.
