Motion carried.</p><p>Safety Incentive Program:</p><p> o Incentive select of winners: Rose Patch Pifer, Clark Kerr at HS Evan ball PRHS, Sharon Upchurch KK, Melinda Mullis at DO</p><p> o Everyone in the district receives the links to online quiz.</p><p> o Safety Incentive Program Draft for review:</p><p>. Monthly safety topic online quiz, drawing of 5 members a month for $20 gift card</p><p>. Online safety training $20 gift card drawing</p><p>. Caught in the act (CIA) $20 gift card drawing </p><p>Old Business: 1. Will be looking at injury reports more closely and returning them back if not complete or incorrect. If there was a safety issue, employee may have to do some corrective action to prevent future incidents, i.e. online training.</p><p>2. Purchases for mats have been made now that the budget has been given. Ladders also ordered but paid through from site grant.</p><p>3. Radios on order and will distributed when they arrive.</p><p>4. Reports</p><p> a. Safe school plan reports complete and updated. Site administrator should have.</p><p> b. Review of employee accidents: Custodian, foot injury $3,528. Teacher tripped on uneven concrete $3,528 Slip and fall, orange peel, $3528 Custodian, foot got caught and fell into split, $1,578.33 Teacher fell foot caught on chair, $3,528 Doing corrective action have to review Student ran into staff’s knee $3,528 Total: $19,218.33</p><p> c. Review of safety inspections from all site Meeting w/ principals and were given 30 days to correct issues. Then safety inspector will go back to check.</p><p> d. Presentation by Raptor: Visitor Manager Management Software (School Entry Control System) o Assures name is real – names stored digitally. Scans IDs, takes pictures and information from ID and does a background check</p><p> o Knows who is on campus in case of emergency</p><p> o Badges – scan legal ID card for first name, last name, and date of birth</p><p>. Names checked against sexual offender data base with visuals. Badge provided if legitimate and designated “w/escort” depending on situation</p><p>. Custom data base of private alert issues. i.e. former employee issues, custody issues, threat history. No badges and notifications sent out.</p><p> o Web base solution – run off the cloud only works with PCs</p><p>. Visitor module – Knows who’s checked in and where on campus. Once scanned in, if a concern, picture will come up on page. It will also check aliases. If any, notifications will be sent to people who need to be alerted. District only. . Student module – Checks in tardies, lists approved pick up folks w/picture, gives most updated custody situation, and reports when check outs are becoming a habit. </p><p>. Faculty modules - Tracks district employee’s whereabouts, ie. technology staff that go from school to school.</p><p>. Volunteer module – registers, creates approval list, manages volunteers, events throughout school district.</p><p> o Silent emergency notice on software, not 911 but will call dispatch to notify necessary people.</p><p> o Can check in and don’t need to print a badge</p><p>. Next year volunteers will have IDs with pictures</p><p> o Will be funded with some grants, very cost effective. Approx. $500. a year.</p><p> o Numerous reports can be generated.</p><p> o Training for less than 15 schools done remotely and through webinar training. Maybe could send out a trainer since we are one of first to use this program in this area.</p><p>New Business:</p><p>1. New IIPP Illness and Injury Prevention Plan to be reviewed by committee</p><p>2. New return to work program to be reviewed by committee</p><p>3. New funding request forms to be reviewed by committee</p><p>4. Funding Request from sites: </p><p> o Pat Butler 4 radios, 2 hats no charge, Jason already has</p><p> o ACE requests 3 first aid kits, one at Pifer, BS, and VP. Cost is $99 each plus tax </p><p> o First aid kits for all sites to be kept in office for staff, Cost $1100 plus tax. Motion made by Linda and 2nd by Wendy. Motion carried.</p><p> o VP 6 non-skit floor mats at $30 each. Motion made by Wendy and 2nd by Irma. Motion carried.</p><p> o BS 4 floor mats 60 plus tax. Motion made by Nancy and 2nd by Laura. Motion carried.</p><p>5. Not all sign in sheets have been turned in by Flamson, Peterson, and Pifer.</p><p>6. New budget for safety committee. Originally started with $2500. New budget includes SIPE’s premium rebate of $9860. Funds going to safety committee. This rebate based on submitted documentation. Could have been twice as much more if all the paper work had been completed. </p><p>7. At next meeting, follow up on work order submitted by PB for window on faculty door, a safety issue for staff. </p><p>Adjourn: </p><p>Motion to adjourn by Linda Bacigalupo and 2nd by Laura Eras.
