<p>A guide to when to use the Health Assessment Addendums </p><p>To ensure specific hazards and risks are screened as effectively as possible, it is necessary to supplement the HM30 Health Assessment Questionnaire Health Questions with addendums for certain roles. </p><p>The following addendums will be automatically sent to the employee if you have ticked ‘yes’ to a risk/activity that has been starred.</p><p>190216 Public Page 1 of 8 HM301 Addendum – Working in safety critical environments, confined spaces, at heights and/or vocational driving</p><p>Safety critical tasks will include those where if sudden incapacity developed it could put either the employee, co workers or members of the general public at risk:</p><p> Working at heights ( above a 3 step step-ladder)  Working with unguarded or moving machinery  Working with live electricity  Working over open water  Working alone  Vocational driving (see below)  Waste disposal or recycling operatives</p><p>This addendum also includes those working in confined spaces such as tanks, kilns, or underground etc.</p><p>Vocational driving includes driving a motor cycle, car, lorry, passenger carrying vehicle, fork lift truck, train or automated manual handling equipment for work.</p><p>190216 Public Page 2 of 8 HM302 Addendum – Working with vulnerable adults or any children</p><p>Work involving adults who may be deemed vulnerable includes all inpatients or those in sheltered housing or care centres.</p><p>This addendum should be used for individuals working with or in the vicinity of any children (not only vulnerable children).</p><p>190216 Public Page 3 of 8 HM304 Addendum – Laboratory workers and those working with skin / respiratory sensitisers / laboratory animals</p><p>Where an individual will be working in a laboratory environment where they will be in contact with GMOs Level 1, GMO's Level 2, viruses, micro-organisms, pathogenic organisms/tissues or with biological agents / chemicals or with skin irritants/sensitisers (*) or Working with respiratory sensitizers/laboratory animals*</p><p>Anyone working with skin sensitisers as part of their role should also complete this questionnaire.</p><p>Additional questions may be needed and this requirement will be driven by the risk assessment. Please speak to your CMO if necessary.</p><p>190216 Public Page 4 of 8 190216 Public Page 5 of 8 HM305 Addendum – Night workers</p><p>A night-worker is defined as one who regularly works for at least 3 hours between the hours of 2300-0600. </p><p>190216 Public Page 6 of 8 HM306 Addendum - Addendum for Magnetic Resonance Imaging Exposure</p><p>Where an individual may be required to enter MRI areas. </p><p>190216 Public Page 7 of 8 HM307 Addendum – Food handlers</p><p>This should be used for all employees who will handle, process and serve food during their working day.</p><p>190216 Public Page 8 of 8</p>
