A guide to when to use the Health Assessment Addendums

To ensure specific hazards and risks are screened as effectively as possible, it is necessary to supplement the HM30 Health Assessment Questionnaire Health Questions with addendums for certain roles.

The following addendums will be automatically sent to the employee if you have ticked ‘yes’ to a risk/activity that has been starred.
