<p> “ TICKET TO PLAY”</p><p>All Wake Forest HS athletes trying out for any sport must have all paperwork completed with appropriate signatures and turned in prior to tryouts in order to receive a “TICKET TO PLAY”.</p><p> The “TICKETS TO PLAY” will be issued in the Commons Area of the Cafeteria at the High School for all Winter Sports on the following dates:</p><p>Tuesday, Oct. 27th -- All three lunches (A, B, and C)</p><p>Wednesday, Oct. 28th -- All three lunches (A, B, and C)</p><p>Thursday, Oct. 29th -- All three lunches (A, B, and C)</p><p> Necessary paperwork:</p><p>1. WCPSS Pre-Participation/Physical form (Current Physical Exam) **Need all 5 pages from the WCPSS Document signed, initialed, & dated**</p><p>2. WFHS Code of Ethics</p><p>3. NCHSAA Athletic Eligibility and Authorization Statement</p><p>4. WFHS Student Athlete’s Travel Form</p><p>5. OSNC Impact Concussion Form</p><p> Anyone trying out for a Winter Sport must have a physical that will not expire until after the season. For the fall season the exam expiration date will be after March 4, 2016.  It is the athlete’s responsibility to get his/her “TICKET TO PLAY” and turn it in to the coach on the first day of tryouts/practice.</p><p>IT IS THE PARENT(S)/ATHLETE’S RESPONSIBILITY TO HAVE ALL FORMS COMPLETED. </p><p>Forms will not be accepted unless they are COMPLETED</p><p>If you have any questions, please feel free to contact the Athletic Director: Mike Joyner at [email protected]</p>
