All Wake Forest HS Athletes Trying out for Any Sport Must Have All Paperwork Completed

“ TICKET TO PLAY”

All Wake Forest HS athletes trying out for any sport must have all paperwork completed with appropriate signatures and turned in prior to tryouts in order to receive a “TICKET TO PLAY”.

 The “TICKETS TO PLAY” will be issued in the Commons Area of the Cafeteria at the High School for all Winter Sports on the following dates:

Tuesday, Oct. 27th -- All three lunches (A, B, and C)

Wednesday, Oct. 28th -- All three lunches (A, B, and C)

Thursday, Oct. 29th -- All three lunches (A, B, and C)

 Necessary paperwork:

1. WCPSS Pre-Participation/Physical form (Current Physical Exam) **Need all 5 pages from the WCPSS Document signed, initialed, & dated**

2. WFHS Code of Ethics

3. NCHSAA Athletic Eligibility and Authorization Statement

4. WFHS Student Athlete’s Travel Form

5. OSNC Impact Concussion Form

 Anyone trying out for a Winter Sport must have a physical that will not expire until after the season. For the fall season the exam expiration date will be after March 4, 2016.  It is the athlete’s responsibility to get his/her “TICKET TO PLAY” and turn it in to the coach on the first day of tryouts/practice.

IT IS THE PARENT(S)/ATHLETE’S RESPONSIBILITY TO HAVE ALL FORMS COMPLETED.

Forms will not be accepted unless they are COMPLETED
