Employee evaluates work performance in relation to the current job requirements and completes the attached form. </p><p>Step 2. The completed form should be forwarded to the supervisor by time supervisor designates.</p><p>Step 3. Supervisor reviews self-appraisal and writes comments in section ASupervisor Comments.@ </p><p>Step 4. Employee and supervisor meet to discuss the appraisal and to review expectations for the upcoming year. </p><p>Step 5.
