<p> Sleep Medicine Communication Plan for Owatonna Clinic DRAFT April 6, 2009</p><p>Situation Analysis</p><p>Steve Kubas, M.D., sleep medicine specialist, has provided care at the Owatonna Clinic since 2004. Dr. Kubas is trained in sleep medicine and evaluates patients and interprets sleep lab results. He comes to Owatonna from the Austin Medical Center’s Sleep Medicine Department – part of the Mayo Health System. Dr. Kubas is board certified by the American Board of Psychiatry. He received his medical degree at the University of North Dakota School of Medicine, Grand Forks, and completed his residency at Mayo Clinic, Rochester. His presence allows Owatonna Clinic to continue to provide a service and patients can consult with a sleep medicine physician and receive treatment in Owatonna locally. Dr. Kubas sees patients one half day a week at Owatonna Clinic’s 26th Street clinic. Owatonna Clinic has partnered with Austin Medical Center to make this service locally available to our patients Sleep studies are conducted at Austin Medical Center and Owatonna Hospital. Consultations and sleep studies have declined during the past 9 months.
