Booking forms must be completed in full and submitted in accordance with the purchasing procedures of the delegate’s employing organisation. 2. If the purchaser requires CESG to quote a valid purchase order when issuing invoices, please include this with the completed booking form. 3. Booking forms must be submitted by the payment authoriser. 4. Please submit your completed booking form by 10th October 2016. 5. The booking form may be photocopied for multiple applications, or additional copies may be obtained from www.cesg.gov.uk. 6. The payment authoriser of the delegate’s organisation becomes liable for the full fee or cancellation charge upon CESG’s acceptance of the booking. 7. We can only accept cancellations of bookings from the payment authoriser. 8. We will charge a cancellation fee, as set out below, if bookings are cancelled without suitable substitution being agreed at the time. 9. Invoices are raised after your place is confirmed for the event. Payment must be made within 30 days of receipt of the invoice. Details of payment are included on the invoice. 10. The event fees include meals and refreshments on 2nd-3rd Nov and accommodation on 2nd Nov. 11. We reserve the right to alter the event venue. Should this be necessary, we will inform delegates as soon as possible and give them appropriate directions and guidance. 12. The detailed content of the event is subject to continuous review. We reserve the right to vary content without notice. Should we be forced to cancel the event, our liability will be limited to the refund of fees paid. No liability will be accepted for any consequential loss. 13. If delegates have any special requirements, it is essential to identify this on the booking form at the time of application.</p><p>CANCELLATION TERMS 14. You are permitted to cancel your place at SUAC 2016 without charge within seven days of confirmation. 15. Thereafter, if you are unable to attend, you may nominate a replacement, subject to approval by CESG, at any point up to seven working days prior to the event (at which point substitutions cannot be accommodated). You must submit the name change request in writing to [email protected]. 16. If you are unable to attend SUAC 2016, and are not in a position to transfer your place to another person, then the following refund arrangements apply:</p><p> registrations cancelled on or before 14th Oct 2016 will be refunded 50% of the registration fees;  registrations cancelled after 14th October 2016 will not be eligible for a refund</p><p>DATA PROTECTION - ALL DELEGATES PLEASE NOTE</p><p>By completing and returning the booking form, you give your consent that personal data provided thereon may be used in administration of the event, to inform you of future relevant events, to assist CESG in supporting you and your organisation, to manage enquiries or complaints or for the issue of any documentation you may request. Your personal data will not be used for other purposes and will be lawfully processed according to the Data Protection Act 1998.</p>
