Portfolio of Evidence</p><p>Evidence of the facilitation process should be presented in a portfolio, which is a collection of related items. Your portfolio should contain the following evidence for each learning relationship:</p><p> copy of the individual learning plan</p><p> facilitation activities or project material</p><p> evaluations from participants or the organisation</p><p> evidence of self evaluation leading to improvement of skills; for example, a learning journal</p><p>3. Place the Cover Sheet at the front of your work and organise your assessment tasks in order. 4. Secure your work with a staple or clip. 5.
