<p> Delaware Association of Nurse Anesthetists (DANA)</p><p>DANA Triple Threat Program</p><p>For a minimum contribution of $75.00 each to the DANA-PAC, CRNA-PAC, and the AANA Foundation ($75.00 minimum contribution to each or $225.00 total), you can become a DANA Triple Threat.</p><p>For Triple Threat Program verification purposes, please mail 3 separate checks (1 for the DANA-PAC, 1 for the CRNA-PAC, and 1 for the AANA Foundation) to Ron Castaldo, CRNA (address below).</p><p>Please make checks for the DANA-PAC out to “DANA-PAC.”</p><p>Please make checks for the CRNA-PAC out to “CRNA-PAC.”</p><p>Please make checks for the AANA Foundation out to “AANA Foundation.”</p><p>In addition to your 3 checks, please complete the member information form (page 2 of this attachment). Include the completed member information page in your envelope, and mail your 3 checks and completed member information form to Ron Castaldo, CRNA.</p><p>Mr. Castaldo will mail all checks to the appropriate addresses.
