DANA Triple Threat Program
For a minimum contribution of $75.00 each to the DANA-PAC, CRNA-PAC, and the AANA Foundation ($75.00 minimum contribution to each or $225.00 total), you can become a DANA Triple Threat.
For Triple Threat Program verification purposes, please mail 3 separate checks (1 for the DANA-PAC, 1 for the CRNA-PAC, and 1 for the AANA Foundation) to Ron Castaldo, CRNA (address below).
Please make checks for the DANA-PAC out to “DANA-PAC.”
Please make checks for the CRNA-PAC out to “CRNA-PAC.”
Please make checks for the AANA Foundation out to “AANA Foundation.”
In addition to your 3 checks, please complete the member information form (page 2 of this attachment). Include the completed member information page in your envelope, and mail your 3 checks and completed member information form to Ron Castaldo, CRNA.
Mr. Castaldo will mail all checks to the appropriate addresses. Again, for verification purposes, please do not mail the checks out separately. Please mail the 3 checks and completed member information form to Ron Castaldo, CRNA at:
