<p> How to Write a Customer Order Form If you have a computer system please use the Order Pro software, see CIA Training site for more information. </p><p>Customer Order Form</p><p>1. List the titles ordered at retail price plus customer specials.</p><p>2. Total up books ordered.</p><p>3. Multiply #1 by 8% and write in s/h due.</p><p>4. Add #1 and #2 and write in total for #3</p><p>5. If you are in a state that does not charge sales tax on s/h (see bolded print on line #1), multiply Total #1 by your local sales tax rate and fill in #4 with tax due. For all other states, multiply subtotal #3 by local sales tax rate and fill in #4 with tax due.</p><p>6. If customer would like to have their order delivered directly to their home, add extra $5 for Direct Delivery charge.</p><p>7. Add #3, #4, #5 for Total Due</p><p>Continued on page 2 2</p><p>* If the order exceeds $10 the customer can use Master Card, Visa or Discover Card. Check the card # and expiration date against their card.</p><p>* Special Delivery customers’ orders can be shipped direct to an address other than the address to which the main order is being shipped. There is a $5 fee in addition to 8% shipping & handling. Complete the customer order form as above, adding $5 at STEP 5. Make sure the Special Delivery address is completed and legible.</p><p>* Include the customer order form with the summary order form only if payment is charged or special delivery is required.</p>
