The administration of doxifluridine was interrupted for grade 3 neutropenia or grade 2 thrombocytopenia or worse until the resolution to at least grade 2 neutropenia, or grade 1 thrombocytopenia with weekly follow-up. If the toxic effects did not improve with 2 weeks of interruption, treatment was discontinued permanently. For grade 3 non-hematologic toxicities (except alopecia, nausea, or vomiting), treatment was interrupted and could be restarted on the recovery to at least grade 1 with 25% of dose reduction. If toxicity recurred at reduced dose, treatment was discontinued permanently. For the patients with moderate renal dysfunction (serum creatinine 1.5 ~ 3.0 mg/dL), cisplatin was administered with reduced dose of 50% and discontinued for further impairment.

2 Supplementary table 1: Site of first recurrence* Site Mf (n=258) iceMFP (n=263) P value No. of patients Total no. of recurrences 118 (46%) 94 (36%) Locoregional sites 15 (6%) 11 (4%) 0·43 Distant sites 103 (40%) 83 (32%) 0·05 Hematogenous 35 (14%) 25 (10%) 0·17 Peritoneum 60 (23%) 44 (17%) 0·08 Lymphatic 41 (16%) 29 (11%) 0·12 * Recurrence at more than one site in some patients

3 Supplementary table 2: Surgery-related complications Complications Mf (n=258) iceMFP (n=263) No. of patients Total no. of complications 36 27 Wound problem 11 9 Ileus 9 10 Intraabdominal abscess 12 3 Bleeding Intraluminal 0 2 Intraperitoneal 1 0 Anastomotic leakage 0 2 Anastomotic stenosis 1 0 Pneumonia 1 0

4
