A student information sheet about the survey that you can hand to each classroom student at the end of training. The information sheet is the first attachment. (page 3) 2. A follow-up email that we suggest that you send to students the day after they complete the course. The copy for the email is the second attachment. (page 5) You will need to add information on both the information sheet and the emails before you distribute them to the classroom students.

To thank each student for completing the survey, each will be entered into a monthly drawing to win one of two $300 VISA Gift Cards. The incentive in May for two randomly chosen instructors who successfully encouraged students to participate in the online research will also be a $300 VISA Gift Card!

We know that this feedback will be invaluable as we strive with you to best prepare students to properly deliver emergency cardiovascular care, CPR and/or first aid.
