<p>Format for Excuse Letter</p><p>Criteria:</p><p>1. Formal Letter.</p><p>2. Date of absent.</p><p>3. Your Name / Class / Section must be stated.</p><p>4. Parent full name / IC number / Contact number / Signature.</p><p>5. All reason must be Explained in full </p><p> e.g. It’s not approved when you just write: Home </p><p> got things / Sick.</p><p> Approved: There is a fire broke out in my house / I </p><p> am sick due to a fever.</p><p>6. Sickness must be covered with an M.C. (Medical Certificate) (Attached) </p><p> otherwise, inform your Sectional leader on the day / or the day before if </p><p> under home-medication.</p><p>7. Passport must be photocopied if you’re overseas on the day of the </p><p> practice.</p><p>8. Death Certificate must be attached when required</p><p>9. If there is a family function or there are family activities on the day of </p><p> practice, inform your Sectional leader beforehand; not the last minute, </p><p> and wait for the teacher-in-charge’s approval. </p><p>10. All letters must be submitted by the next CO practice.
