<p> INSPECTION DELEGATION STAMP REQUEST CHECK SHEET </p><p>DATE: DELEGATE NAME: </p><p>SUPPLIER NAME / CODE: DELEGATE EMAIL: </p><p>LOCATION: SQA COMMODITY LEAD NAME: </p><p>The following are requirements to be reviewed by proposed Inspection Delegate personnel. Please check off each category to confirm understanding and comprehension of L-3 CPS requirements. Note: Use one (1) check sheet for each delegate request. Return completed form along with completed Inspection Delegation Request Form to the SQA Commodity lead and [email protected]</p><p>1. The Inspection Delegate (ID) is qualified to perform acceptance activities. </p><p>2. The Inspection Delegate has: a. Technical Competence b. Quality Assurance background and experience c. Unrestricted access to L-3 CPS SQA d. Access to the manufacturing process for process verification e. Ability to exercise the responsibility of the position as defined within this document (including a ‘No Ship’ directive) </p><p>3. I understand that a copy of the stamped shipper/packing slip and all supporting inspection data and objective evidence of </p><p> product conformance is to be maintained on file. </p><p>4. I understand that records must be retained for 7 years after Purchase Order Completion </p><p>5. I understand that documents must be available for L-3 CPS/ Government review at any time. </p><p>6. The ID stamp must be kept under delegate control and secured from unauthorized use. </p><p>7. I am to notify my L-3 CPS Commodity Lead immediately if this stamp is lost, stolen, damaged or worn. </p><p>8. I am to notify my L-3 CPS Commodity Lead if I suspect any unauthorized use of this stamp. </p><p>9. I understand that the stamp impression affixed to the shipping documentation is the only form of product acceptance. </p><p>Unstamped items may be returned at L-3 CPS discretion at the Supplier’s expense per the Purchase Order Terms and </p><p>Conditions. </p><p>10. I have reviewed the Supplier Information Package and Quality Clauses located on the L-3 CPS Quality webpage: </p><p> http://www2.l-3com.com/cps/quality.html 11. I understand that even though I have an L-3 ID stamp, I am still required to submit my FAI/FPI documentation to L-3 CPS for</p><p> a pre-review when I am supplying a NEW PART NUMBER to L-3 CPS for the first time. </p><p>12. I understand as the ID Delegate stamp holder, I do not have the authority to stamp off on First Article Inspection (FAI) and </p><p>First Piece Inspection (FPI) requirements. </p><p>13. The ID Delegate stamp cannot be transferred / reassigned to someone else in my company. </p><p>14. I understand that I do not have MRB authority. </p>
