A The Registration Certificate Archive is now closed but you are able to order a certificate online if the event took place in Hertfordshire. (please note the office is open Monday – Friday, so any priority orders made over the weekend will be processed on a Monday and sent out by first class post) (Note that we do not hold records for the London Borough of Barnet). A The Registration office is now closed but you can book an appointment online. Our Web-site address is www.hertfordshire.gov.uk, alternatively I can book you an appointment now. PLEASE NOTE AND MAKE CALLERS AWARE IF THIS APPLIES If the event took place in Hertfordshire, it can be registered in Hertfordshire. If the event took place outside Hertfordshire, they can attend our offices but by declaration, so would get no documents until registered in other area and then documents will be posted to the informants. In this case a cheque book would need to be taken to the appointment to request documents. Q Who can register a birth/attend the appointment? The mother of the baby The father of the baby if he was married to the mother at the time of the birth The mother and the father, jointly, if they are not married to each other at the time of the birth The mother or the father, attending alone, if they were not married at the time of birth, bringing with them an appropriate document signed by the other parent. Enquire about this document at any Registration Office The mother, if she is not married to the father, may register alone leaving out the details of the father. A re-registration can enter the father’s details at a later date or following the marriage of the father and mother For any couple in a Civil Partnership or same sex relationship please contact 0300 123 4045 or further advice on registering Q How long after the birth of my baby do I need to register? A All babies must be registered within 42 days. Q What information will the registrar ask for? A Place and date of the birth Name, surname and sex Parents’ names, surnames and address Places and dates of parents’ birth Date of parents’ marriage or civil partnership Parents’ occupations Mother’s maiden surname Q What do I need to bring with me? A Mother and baby discharge papers or baby’s red book £4.00 for each certificate required at the time of registration (if they come back after the appointment the certificates will be £7.00) Q Do I have to take the baby with me to the appointment? Medical Certificate Cause of Death from a General Practitioner, or Hospital Q What if a death is referred to the Coroner? Paperwork will be required from the Coroner before a registration can take place. Due to a change in our process we are unable to make an appointment until we have the Coroner’s signed paperwork. A member of our team will contact the family once we receive the paperwork. If the family wish to contact us to clarify, they can on 0300 123 4045 during office hours. Q Who can register a death? A relative of the deceased A person present at death The occupier of the house or institution where the death took place, if there is no known relative who is able to register The person who is arranging the funeral, that is the person instructing the Funeral Director Q What information will the registrar need? The date and place of death The deceased’s last usual address The deceased’s full name and surname (and the maiden surname if the deceased was a woman who had married) The deceased’s date and place of birth The deceased’s occupation and the name and occupation of their spouse or civil partner Whether the deceased was getting a pension or allowance from public funds If the deceased was married or a civil partner, the date of birth of the surviving widow, widower or civil partner If readily available the Medical Card or the National Health Service number 9 Q What documents will the Registrar give me on completing the registration? The Registrar will issue you with a form to take to the Funeral Director (in some cases, this will have been issued by the Coroner) which gives permission for burial or cremation often known as the Green Form). You will also be given another form (BD8), which you should take or send to the DSS in respect of the state pensions and benefits. You will also receive a reference number for the Tell Us Once service which will allow you to notify anyone government or council related free of charge without sending off any death certificates.
