Instruction Manual Part 8 Vital Records, Geographic Classification, 2003 Vital Statistics Data Preparation U.S. DEPARTMENT OF HEALTH AND HUMAN SERVICES Public Health Service Centers for Disease Control and Prevention National Center for Health Statistics Hyattsville, Maryland October, 2002 VITAL RECORDS GEOGRAPHIC CLASSIFICATION, 2003 This manual contains geographic codes used by the National Center for Health Statistics (NCHS) in processing information from birth, death, and fetal death records. Included are (1) incorporated places identified by the U.S. Bureau of the Census in the 2000 Census of Population and Housing; (2) census designated places, formerly called unincorporated places, identified by the U.S. Bureau of the Census; (3) certain towns and townships; and (4) military installations identified by the Department of Defense and the U.S. Bureau of the Census. The geographic place of occurrence of the vital event is coded to the state and county or county equivalent level; the geographic place of residence is coded to at least the county level. Incorporated places of residence of 10,000 or more population and certain towns or townships defined as urban under special rules also have separate identifying codes. Specific geographic areas are represented by five-digit codes. The first two digits (1-54) identify the state, District of Columbia, or U.S. Possession. The last three digits refer to the county (701-999) or specified urban place (001-699). Information in this manual is presented in two sections for each state. Section I is to be used for classifying occurrence and residence when the reporting of the geographic location is complete. Section II is to be used for classifying occurrence and residence only when the county is not reported. SECTION I This section contains an alphabetical listing of counties and, alphabetically within each county, incorporated cities of 10,000 or more population and certain towns or townships in Michigan, New England, New Jersey, New York, Pennsylvania, and Wisconsin defined as urban under special rules. These places have either a population of more than 25,000 or a population of 10,000 to 25,000 and a population density of at least 1,000 per square mile. Three-digit place codes (001-699) are assigned within each state to these urban places. Cities that are located in more than one county have a separate code assigned to each part. Each part is shown under its respective county in this section. Also listed alphabetically within each county are inclusions to these urban places. “Inclusions” are names of sections of the cities (including military installations) that may be reported on the vital record; the have the same place code as the urban place of which they are part. The “Balance of County” or “Entire County” is identified by a 3- digit code (701-999), which is used to identify the county of occurrence and the place of residence for cities or places not separately identified. For independent cities and cities that are coextensive (coext.) with the county in which they are located, there is no county code and the place code (001-699) is used for place of occurrence as well as place of residence. SECTION II This section is an alphabetical listing of all incorporated places regardless of population size, census designated places, military installations, and inclusions. For urban places of 10,000 or more population that are located in more than one county, only the code for the part with the largest population is shown. For places with less than 10,000 population and are located in more than one county, the allocation is to the balance of county, which has the part with the largest population. In some states, there is more than one place with the same name located in different counties. All such places are included in this section of the manual with their appropriate place or county code. Separate county codes are needed to apply the coding rule that when no county of residence is given, and there is more than one city with the same name in different counties, the allocation is to the county of occurrence.
