Understanding that as a participant in the 2nd Summer Youth Olympic Games in Nanjing, from 16 to 28 August 2014, I am participating in an exceptional event and, in consideration of the acceptance of my participation therein, I agree to be filmed, televised, photographed, identified and otherwise recorded during the Nanjing 2014 Summer Youth Olympic Games under the conditions and for the purposes now or hereafter authorised by the International Olympic Committee (IOC) in relation to the promotion of the Youth Olympic Games and the Olympic Movement, including for use to such effect by any third parties authorised by the IOC. 2. I declare that I have read and that I have full knowledge of the provisions of the Olympic Charter currently in force and that I agree to comply with all such provisions and, in particular, with the provisions of the Olympic Charter regarding eligibility for the Youth Olympic Games (including Rule 40 and its Bye-law), the mass media (Rule 48 and its Bye-law) and the prohibition on forms of publicity or propaganda on persons or on clothing and equipment worn or used at the Youth Olympic Games (Bye-law to Rule 50). I further agree to comply with the provisions of the World Anti-Doping Code in force at the time of the Nanjing 2014 Summer Youth Olympic Games as well as the IOC Anti-Doping Rules applicable to the Nanjing 2014 Summer Youth Olympic Games, including in particular the blood and urine sample collection process and I note the rights and responsibilities which apply upon notification as set out in Appendix 3 of the IOC Anti-Doping Rules. I also agree to comply with the IOC Code of Ethics, in particular to not participate in, support or promote betting related to the Youth Olympic Games, and to comply with the IOC Social Media, Blogging and Internet Guidelines for persons accredited at the Nanjing 2014 Summer Youth Olympic Games. I further agree that third parties may transfer to the IOC information pertaining to betting activities in relation to the Youth Olympic Games. 3. I declare that I have full knowledge of all such provisions and that all provisions applicable to my participation in the Nanjing 2014 Summer Youth Olympic Games have been specifically brought to my attention by my National Olympic Committee (NOC) and/or by my national sports federation or by the international sports federation and are specifically approved by me. 4. I agree that all photographs and moving images taken by me at the Nanjing 2014 Summer Youth Olympic Games, including those of athletes competing within any Olympic venues, shall be used solely for personal and non-commercial purposes and in accordance with the IOC Social Media, Blogging and Internet Guidelines for persons accredited at the Nanjing 2014 Summer Youth Olympic Games, unless prior written consent is obtained from the IOC. 5. I acknowledge and agree that : a. I participate in the Nanjing 2014 Summer Youth Olympic Games at my own risk and that I will take all reasonable measures to protect myself from the risks of participation ;and b. I am responsible for all property I bring into the Youth Olympic Games sites and that the Nanjing 2014 Summer Youth Olympic Games Organising Committee (NYOGOC) and the IOC shall have no responsibility for any loss or damage to this property. 6. I irrevocably release NYOGOC and the IOC (and their respective members, directors, officers, employees, volunteers, contractors or agents) from any liability (to the extent permitted by law) for any loss, injury or damage that I may suffer in relation to my participation in the Nanjing 2014 Summer Youth Olympic Games. 7. In respect of information about myself (my ‘Personal Data’), I agree : a. to my Personal Data being collected by the IOC and NYOGOC and to such data being stored and used by the IOC (including being stored on the IOC’s servers in Europe) and NYOGOC and, where necessary, by third parties for the purposes of, and to the extent necessary in relation to, organizing the Youth Olympic Games, facilitating my participation therein and presenting competing athletes to the public ; b. to the IOC storing and using my personal data after the conclusion of the Youth Olympic Games, for historical and statistical purposes ; c. to the transfer of my Personal Data to the authorities of the People’s Republic of China and by them to third parties, and to the processing of such data and potentially any relevant sensitive personal data, as is necessary for security and other background checks by the authorities of the People’s Republic of China in order for me to gain the necessary accreditation for the Youth Olympic Games ; d. that the IOC, NYOGOC and other third parties, including law enforcement and border services agencies, may share amongst themselves and with third parties my Personal Data for the purposes of investigating and/or prosecuting breaches of any of the provisions, rules or bye-laws referred to in paragraph 2 above (for example, breaches of anti-doping and/or betting rules) ; e. to the collection and use of Personal Data and statistics in IOC approved research projects (for example, athlete biographies, questionnaires, filming, measures, medical encounters etc) conducted during and after the Youth Olympic Games; and f. to my Personal Data being used in any other way to which I provide my express consent to the IOC and/or NYOGOC. 8. I agree that any dispute, controversy or claim arising from, in connection with, or on the occasion of the Youth Olympic Games, not resolved after exhaustion of the legal remedies established by my NOC, the international federation governing my sport, NYOGOC and the IOC, shall be submitted exclusively to the Court of Arbitration for Sport (CAS) in Lausanne, Switzerland, for final and binding arbitration in accordance with the Code of Sports-related Arbitration. The seat of arbitration shall be at Lausanne, Switzerland. The CAS shall rule on its jurisdiction and shall have the exclusive power to order provisional and conservatory measures. The decisions of the CAS shall be final, binding and non-appealable. I shall not, and I hereby waive my right to, institute any claim, arbitration or litigation, or seek any other form of relief in any other court or tribunal. 9. I acknowledge and agree that my participation in the Youth Olympic Games is conditional on my valid acceptance of and my compliance with each and all of the provisions of this Entry/Eligibility Conditions Form (referred to as “Entry Form” in the Olympic Charter). 1 The NOC hereby certifies and guarantees that all the relevant rules, including all those referred to above, have been brought to the notice of the participant and that the NOC has been authorised by the national sports federation concerned to sign this Entry/ Eligibility Conditions Form on the latter’s behalf, with the approval of the relevant international federation. Eligibility Code To be eligible for participation in the Olympic Games, a competitor, coach, trainer or other team official must comply with the Olympic Charter, including the conditions of eligibility established by the IOC, as well as with the rules of the international federation concerned as approved by the IOC, and the competitor, coach, trainer or other team official must be entered by his NOC. The above-noted persons must : - respect the spirit of fair play and non-violence, and behave accordingly; and - respect and comply in all aspects with the World Anti-Doping Code. Bye-law to Rule 40 1. Each international federation establishes its sport’s own eligibility criteria in accordance with the Olympic Charter. Such criteria must be submitted to the IOC Executive Board for approval. 2. The application of the eligibility criteria lies with the international federations, their affiliated national federations and the NOCs in the fields of their respective responsibilities. 3. Except as permitted by the IOC Executive Board, no competitor, coach, trainer or official who participates in the Olympic Games may allow his person, name, picture or sports performances to be used for advertising purposes during the Olympic Games. 4. The entry or participation of a competitor in the Olympic Games shall not be conditional on any financial consideration. 48. Media Coverage of the Olympic Games 1. The IOC takes all necessary steps in order to ensure the fullest coverage by the different media and the widest possible audience in the world for the Olympic 2. All decisions concerning the coverage of the Olympic Games by the media rest within the competence of the IOC. Bye-law to Rule 48 1. It is an objective of the Olympic Movement that, through its contents, the media coverage of the Olympic Games should spread and promote the principles and values of Olympism. 2. The IOC Executive Board establishes all technical regulations and requirements regarding media coverage of the Olympic Games in an IOC Media Guide, which forms an integral part of the Host City Contract. The contents of the IOC Media Guide and all other instructions of the IOC Executive Board are binding for any and all persons involved in media coverage of the Olympic Games.
