GROWING UP AT HOME, NOT THE HOSPITAL

NAC believes that every child deserves to grow up safely in a home, not a hospital bed. For 35 years, we have been providing intensive case management and family support services through our Prevention Programs to ensure that at-risk children with medical complexities do not enter the foster care system, are safe from abuse and neglect, and don’t remain hospitalized beyond medical need. Our latest prevention program, Healthy@Home, is the only MARIA & YASMEEN’S STORY program in New York City which helps medically complex children living in hospitals to go home to their families for good. Maria just turned two – a birthday her mother Yasmeen thought she would never spend with her daughter. For the first year of Maria’s life she lived in a hospital. She was born with microcephaly and a genetic condition called velocardiofacial syndrome, causing heart and kidney problems. Maria is non-ambulatory, uses a feeding tube and has a tracheostomy. At night she must be connected to a ventilator. While Maria was in the hospital, Yasmeen was caring for her older sister at home. For the young mother, Maria’s medical care nac’s prevention programs served seemed overwhelming and she didn’t think she would PREVENTION PROGRAMS: ever be able to care for her daughter at home. It is often said that it takes a village to raise a child. At NAC, this is no exception. When a child cannot remain safely at home, NAC steps in with Foster Care & Adoption services tailored to meet the extraordinary needs and challenges of medically complex children. From our nurturing foster parents to our team of compassionate master’s-level Social Workers, Behavior Specialists, Supervisors, Medical and Mental Health Clinicians, Education Specialists, Family Support Specialists, and B2H Health Care Integrators and Waiver Service Providers, everyone works together to ensure that children have the very best home care possible while concurrently planning for reunification or adoption. As a result, our children spend less time in foster care and virtually never age-out without a family to call their own. At NAC, we make sure that every child has a forever family.

The brothers entered foster care at the ages at the age of 14 and 8 after their birth mother was unable to meet all of their extraordinary medical needs. Both boys are non-ambulatory, care achieved or are on their way to achieving non-vocal, have significant developmental delays, and experience seizures. Finding a foster family to care for them together was a challenge. That was until Roshel, a long-time foster parent at NAC, stepped up and said she would care for them. Roshel is a former home health aide and has experience caring for permanency because of our B2H program. people with significant medical conditions. She immediately fell in love with the boys and knew it was her calling to care for them and become their forever mother. With the help and support of NAC’s foster care team, B2H team, nurses, pediatrician, and educational specialists, Jon and Justin were adopted by Roshel in May 2016! The newly opened Vivian & Roger Farah Comprehensive Healthcare Center offers an array of innovative health services through a licensed Article 28 Medical Clinic, licensed Article 31 Mental Health Clinic, and Mobile Medical Clinic. The child-focused and family-friendly center provides pediatric care and care coordination for all NAC children, as well as, mental health assessment, psychotherapy, and a host of alternative interventions. We believe that quality healthcare is the key to improving the well-being and long-term stability of NAC children. That is why our healthcare team, consisting of a Developmental Behavioral Pediatrician, Pediatric Nurse Practitioners, Nurses, Psychiatrists, Psychologists, and Psychotherapists, work together to ensure that our children and families receive the best and most compassionate care possible.

We believe that NAC children have the potential to excel in school, graduate from college, find a job, and live independent, full lives. That is why we go the extra mile to ensure that our children have the tools and opportunity to achieve academic success. NAC has a team of 9 master’s-level Education Specialists who advocate for children’s needs and work with their teachers, schools, and parents to ensure they have the support to stay in school and graduate. Programs like Early Literacy, Tutoring, and NAC’s College Bound Program support and encourage NAC kids to reach their potential. ASHLEY’S STORY

Ashley, age 18, is an inspirational young woman who always had dreams of going to college. Her determination is extraordinary especially given the adversity she has faced in her short life. At the age of two Ashley was taken away from her mother and lived with her grandmother until she passed away five years later. Ashley then lived with relatives and was eventually placed in foster care at the age of 12. Three years later, she went to live with a foster family who nac’s educational team worked with wanted to adopt her and still lives with this family. Because the needs of NAC children are complex and overlapping, our services don’t stop at the basics. We do everything possible to help medically complex children, their siblings, and parents succeed.
