Service Overview A 20 bed unit providing 24 hour patient care for patients who require Disability rehabilitation and support following a stay in hospital or because of a physical change in their condition which means that they cannot care for themselves at home.

The Unit is staffed by a dedicated team of nurses, therapists and health care professionals, working closely with local GPs and Social Care teams.

A programme of rehabilitation is agreed to help achieve all essential goals identified for discharge. A stay will be for a maximum of four weeks but may be considerably shorter

NHS Service Overview Episodes of rehabilitation can be extended across the recovery pathway as individual needs change. The service is offered to individuals whose

needs are complex and cannot be met by local services. This is achieved

through assessment and identification of clinical need. The service is

delivered at two levels: - Brain

1) Case management, linking in with commissioning functions.

Placement with a wide range of providers is facilitated and managed by the team on behalf of NHS Bedfordshire. 2) Community rehabilitation, where possible episodes of rehabilitation

closer to home are provided including multi-disciplinary interventions Disability and co-ordination with other support services. Input is given at the patient’s current location (home or care home) or elsewhere at community settings. The service comprises Occupational Therapists, Psychologists and Rehabilitation Support Workers. Charity working closely with NHS High Level Service Description The wide range of services promotes rehabilitation/enablement helping our clients to build on their existing skills (cognitive, physical, social and emotional) and in turn increase their confidence and self-esteem.

Headway Bedford provides continued rehabilitation/enablement at their Bedford base for clients living at home, where they may have

received a period of rehabilitation from visiting health professionals, Brain and are now ready to engage with activities outside home.

The model delivers a transition service between home and the rest of the community during which there is increased emphasis on personal choice; encouraging confidence and developing independence. The active presence of the many different types of people at the centre - staff, volunteers, other clients – enables practise of skills learned in a safe environment, guided and supplemented by professional therapists and key workers.

While attending Headway, clients will be encouraged, supported and enabled to engage in activities which help them keep focussed on increasing their ability to function purposefully in the rest of the community.

The service is a blend of services delivered by health professionals (OT, speech therapy, physiotherapy) and other staff and volunteers with a

variety of backgrounds and experience. This provides clients with access to a range of diverse services under one roof to enable their recovery. It includes an educational focus which allows clients to learn new skills, including introduction to leisure opportunities such as digital photography, art, pottery and cookery. Private Service Overview Park House is a 12 bed neuro rehabilitation unit for male and female adults over 18 years of age. Its aim is to provide high quality neuro rehabilitation to patients with acquired brain injury and associated physical disability or cognitive disabilities. Park House offers a multi professional approach to rehabilitation in 6 to 16 week packages, which is led by a Consultant in Rehabilitation Medicine. The team of neuro physiotherapy, occupational therapy, speech and language therapy and psychology, provide comprehensive assessment of a person’s cognitive, physical, sensory and emotional functions and the

impact they have on a person’s activities and participation in daily life Brain skills.

Park House specialises in community integration. Much of the rehabilitation is carried out in the community where possible, as situational rehabilitation has been found to be highly effective in

maintaining motivation and engaging patients in the rehabilitation Disability process. For those who are unable to return home Park House team will work with local providers in finding suitable accommodation which able to meet their on-going needs. Park House also has a small number of long- term beds for those residents with more complex needs and who require on-going support from the team. Private Service Overview Askham Court is a 12 bedded unit which has been designed specifically for patients seeking rehabilitation input to assist them to manage living as independently as possible following traumatic injury to brain or spine, or traumatic amputation. All rooms are en-suite, with overhead tracking and independent access. The Service helps patients achieve Amputee their goals and supports patients who present with highly complex physical disability needs and/or complex cognitive and behavioural needs. The service provides a range of therapies on site including

Physiotherapists, Occupational Therapists, Speech and Language Brain therapy, Hydrotherapy, Psychology and Medical support alongside our daily activities, nursing, and care services. The multidisciplinary team

are supported and guided by Consultants in Rehabilitation Medicine.

Askham Court is equipped and staffed to work with patients whose

primary needs are complex, including tracheostomy/ventilation, Disability complex spasticity, prolonged disorders of consciousness, and who require specialist medical and complex therapy interventions. In

addition, patients whose primary needs are for moderately severe cognitive and behavioural management which may include behaviours

that present moderate risk, those requiring close monitoring and part Spinal time 1:1 supervision, who present with neuropsychiatric issues and require the design and delivery of structured rehabilitation

programmes.

The unit provides fixed term rehabilitation programmes as well as bespoke packages for people with long term neurological conditions who seek to maintain as much function as possible as their condition evolves.

The service provides a range of therapies on site including Physiotherapists, Occupational Therapists, Speech and Language therapy, Hydrotherapy, Psychology, neuro-psychology and medical

Askham Place is equipped and staffed to meet the needs of patients whose primary requirements are for highly complex physical care,

including tracheostomy/ventilation, complex spasticity, prolonged Brain disorders of consciousness, and/or moderately severe cognitive and behavioural management arising from traumatic brain injury. These

include behaviours that present moderate risk, those requiring close monitoring and part time 1:1 supervision, who present with neuropsychiatric issues and require the design and delivery of

Service Overview Headway Cambridgeshire provides a specialist service to people with Brain brain injury and the families/carers. Headway provides practical help, support, guidance, signposting, expertise, information and specialist services in our headquarters in Cambridge, in the patient’s home, in the community and in the two hubs in St Neots and Peterborough. Clients have access to counselling and occupational therapists that can help with vocational support, adaptive equipment and activities of daily living. There is an in-house gym at the Cambridge HQ. A programme of courses, workshops and training opportunities is run throughout the year. The desired outcomes for our clients are personal progression, reablement, social inclusion and increased self-confidence.

Private Service Overview The Marbrook Centre is a 54 bedded unit which offers fast and slow stream rehabilitation in packages of 6-16 weeks for patients who have sustained a brain injury, spinal cord injury, a stroke or onset of other

neurological conditions resulting in physical disability.

The multidisciplinary team of nurses and carers, physiotherapists, Brain occupational therapists, speech & language therapists, a psychologist and therapy support workers are supported and guided by a Consultant

in Rehabilitation Medicine. Input from the team is goal orientated and focused on achieving the client’s optimum potential. Discharge home is the goal wherever possible.

Disability Many rooms have overhead tracking for hoists and there are dedicated rooms onsite for more complex care which feature piped oxygen to enable care of a patient with a tracheostomy. There is a transitional flat

The clinical team includes clinical psychologists, speech and language therapists, occupational therapists, an assistant psychologist and a

Goal directed interventions include:

a. Individual holistic assessments to provide a formulation and define Brain client centred, goal directed, individually designed rehabilitation plans b. Intensive holistic interdisciplinary rehabilitation programme (typically

lasting 18 weeks) Intensive phase (first six weeks) includes group therapy sessions on understanding brain injury, attention and memory, executive functions, communication skills, and mood as well as individual sessions with all disciplines, support group, yoga, and family days. Integration phase (usually 12 weeks) includes individual sessions with all disciplines plus support group, psychological support, and additional options such as yoga, fatigue management, control skills, emotion regulation, perspectives group, and projects c.
