Gavin Lee Rheumatologist-HK Rheumatologist Hong Kong

With the advancement of the technology in ultrasound, it has been employed in different medical specialities to provide point of care. Ultrasound is now playing an increasingly important role in the management of patients with musculoskeletal complaint. Spondyloarthritis (SpA) is an important subgroup of inflammatory arthritis, which should be identified early and be treated accordingly. During the early phase of disease, patients with SpA can just present with peripheral arthralgia or joint swelling (namely, peripheral spondyloarthritis). This poses diagnostic difficulty to clinician, when one tries to differentiate from seronegative rheumatoid arthritis or crystal arthropathy. Some of sonographic findings can provide clues to those differential diagnoses. Sacroiliac joint involvement is hallmark of this group of disease, however, it has been considered to be quite disappointing in using USG in studying this very deep joint. Nevertheless, recent data showed it could be possible to identify joint effusion in this particular joint. Further study would be of great interest. Enthesopathy or enthesitis is an important feature found among patients with SpA, which can be identified by USG. One should be careful in distinguishing this from mechanical enthesopathy. Interesting data has been reported in identifying subclinical psoriatic arthritis, an entity belonging to the family of SpA, among patient suffering from skin psoriasis. Therefore, the application of MSK USG, has been expanding in the field of SpA.
