BOARDING TRAINING BEHAVIOR INFORMATION SHEET

OWNER ______DOG’S NAME ______AGE _____ On leash basic obedience commands to be taught: Sit, Down, Sit/Stay, Down/Stay, Come, Leash Walking, Wait at Doors I am having the following behavior problems with my that I would like addressed during training:  JUMPING ON PEOPLE  ON FURNITURE  MARKING/LEG LIFTING  MOUTHING/PLAYBITING  HYPER/ACTIVE  TOUCH SENSITIVE  DOOR CRASHING  EXCITABLE  FEARFUL  GOING IN TRASH  TIMID/SHY  PULLING ON LEASH  PIDDLING  ON COUNTERS/TABLES  TAKING THINGS Such as:  DIGGING  In fenced yard  On tie out  Left outside for how long?

 HOUSEBREAKING (taught to go outside on grass) or  PAPER TRAINING (taught to go INSIDE on pads) that come for housebreaking or paper training will be crate trained. Dogs that come for housebreaking will also be taught to ring a bell to go outside. When your dog comes home you will be required to keep him/her on watch (actively supervise) or in confinement for at least one week. Your dog must be caught in the act for any/all accidents. (You will need a crate if you do not have one already.)

How often does your dog have accidents?  Daily  Weekly  Monthly Do you have a fenced yard? Do you ever catch your dog in the act? How do you correct your dog? Will your dog relieve itself in front of you outside on grass? Have you done with your dog? What type of crate?  Wire  Plastic How long is your dog left in the crate during the day? Does your dog /whine in the crate? *Does your dog soil the crate? How often?  Daily  weekly When?  Overnight  Daytime *Dogs that soil the crate usually require a longer program.  CHEWING (taught to chew appropriate objects) Owner agrees to provide proper toys when the dog is returned home Do you catch your dog chewing inappropriate objects? How do you correct your dog? Does your dog:  Play with toys by him/herself  Bring toys to you to play  Doesn’t play with toys How often do you play/exercise your dog? How is exercise provided? Where did you get your dog/? How long have you had your dog? What type/brand of food do you feed your dog? How often? Why did you decide to do boarding training? I am having the following behavior problems with my dog which will not be addressed during the boarding training program, but can be addressed in my home:  Food/Object Guarding  Bite/Growling  Dog Aggressive  Other Do you understand the reinforcement and maintenance required to maintain these behaviors when your dog is returned home?

I understand that dogs are individuals with varying abilities, and many of my dog’s behaviors are a result of my past interactions with my dog. I understand that the overall success of my program depends upon my dog’s ability level and history and my strict adherence to the program instructions set forth for me by my trainer when my dog is returned home.
