Direct Primary Care: a Legal and Regulatory Review of an Emerging Practice Model
DIRECT PRIMARY CARE: A LEGAL AND REGULATORY REVIEW OF AN EMERGING PRACTICE MODEL Abstract Direct Primary Care (“DPC”) practices are a type of retainer practice where physicians directly charge patients a periodic fee and avoid any third party fee for service payments. These physicians have been small and overlooked group for many years, but the recent growth of the model demands attention. Previously tacit insurance commissioners have taken notice. Laws enacted by six state legislatures and the Affordable Care Act provides a background from which a legal framework can be developed. This article will articulate “business of insurance” concerns encountered by DPC physicians including recommended contractual provisions to minimize this risk, compare state laws written chiefly to address this concern, consider the DPC provisions of the Affordable Care Act, and briefly consider tax and scope of practice policy implications of the DPC model. A Definition & Introduction A retainer practice model involves a contract between the physician and patient whereby ongoing primary care services are provided in exchange for a periodic fee.1 For the practice to qualify as a direct primary care practice (a subset of the retainer category) the practice must 1) charge a periodic fee, 2) not bill any third parties on a fee for service basis, and 3) any per visit charge must be less than the monthly equivalent of the periodic fee.2 Billing third parties on a 1 Eskew, P. Having played the instru- ment for, then, almost two decades, I thought that a novelist would not, could not, feature specialty instruments such as the C-melody sax if he did not play the horn himself. Once the saxophone had caught my attention, I noticed all sorts of uncommon references that seemed to confirm my hunch that Thomas Pynchon himself played the instrument: McClintic Sphere’s 4½ reed, the contra- bass sax of Against the Day, Gravity’s Rainbow’s Charlie Parker passage. -

Sahalie Historical Note #10: Members of the 1940s Second in my decadal series, this article continues my attempt to document all references to Sahalie Ski Club members ‐‐ in this case, the 1940s. It is thus long and tedious, with lots of footnotes. If you make it through, and notice any names that bring up stories to tell or more details that we’d like to know about, please get in touch! I should note that it is quite likely that many members joined the club earlier than their first mention in the newspaper or other official source. As we fill in more details from family records and other references, I’m sure we will lower the date of first membership for some people. On that note, I should also mention that all of these articles are intended to be drafts or works‐in‐progress. As we learn more, we will keep updating these records to reflect our best understanding of Sahalie Ski Club and the history of skiing in the Northwest. So definitely speak up if you trip over something or have something to add to the story. A brief summary of Sahalie in the 1940s: The early 1940s showed Sahalie Ski Club booming, with the world’s situation looming. Two of Sahalie’s prodigies, Bill Redlin and Carl Neu, led the U.W. ski team to national honors. Bill went on to teach skiing to Army troops in early 1942; Carl enlisted in 1943 after graduating. Other notable ski racers for the club in the early 1940s Bill Redlin and Carl Neu, both former Sahalie members, racing for the U.W. -

Cambridgeshire Guild of Weavers, Spinners and Dyers Letter from the Chair Thank you to everyone who helped make the Exhibition such a success. We had lots of visitors and the comments in the Visitors’ book are very encouraging. The demonstrations were very popular and a lot of children had a go at weaving. Jacqui Fuller and her team did sterling work in the Reading Room providing drinks and homemade cakes as well as overseeing the dye plants that were for sale. Sue Prior produced masses of plants for sale. Paula Armstrong, Beth Parks and Sue Melvin were the ‘hanging group’ , ably assisted by Chris Tucker and Lynn Reynolds on the Friday afternoon. Paula also organised the sales table which looked spectacular. I am indebted to Ann Ruel for facilitating the display of Jan Morris’ work. It was lovely to see her weaving again and her family were touched and delighted that we were able to do this. The range and quality of the work on display was breath taking and the ‘Challenge’ was fascinating to see how everyone had interpreted their part of the picture. Thank you. Sue Letter from the Newsletter Editor Thank you to everyone who provided content for this newsletter. So many folks have come together to provide book reviews, meeting summaries, recipes and announcements, that I mainly played around with adjusting colours for this issue! I do love design and I am glad to be able to combine two of my creative outlets in one place. I will strive to keep a fairly consistent layout, but will likely change up a few things each issue.
