RIGHT PLACE, RIGHT TIME BETTER TRANSFERS OF CARE: A CALL TO ACTION ‘Doing the obvious thing is the radical thing’ INTRODUCTION There is no simple solution to delays in transfers of care: no one individual to blame nor a magic bullet that will solve everything. Everyone in the health and social care system needs to act: commissioners and providers, physical and mental health, national and local organisations. However there are a number of practical steps we can take to understand the root causes of delayed transfers of care and build the partnerships needed to tackle them.
