1. Application of Tariff

1.1 This tariff contains regulations, rates and charges applicable to the provision of Carrier Common Line, End User Access, Switched Access and Special Access Services, and other miscellaneous services, hereinafter referred to collectively as service(s), provided to customers by the following telephone companies of the Verizon Telephone Companies as issuing carriers: Verizon Pennsylvania Inc., Verizon New Jersey Inc., Verizon Delaware LLC, Verizon Washington, D.C. - DS3 Switched Access Entrance Facilities channel terminations (electrical and optical) as described in Section 6.9.1 following - DS3 High Capacity Special Access Service channel terminations (electrical and optical) at a primary premises as described in Section 7.5.9 following.

Before the first billing period of each month, the Telephone Company will count all eligible DS3 Switched Access Entrance Facility channel terminations that are in service between the same two locations (e.g., a customer-designated premises and the same serving wire center). If that customer-designated premises is a primary premises for Special Access Service as defined in Section 7.4.1 following, the Telephone Company will also count all eligible DS3 High Capacity Special Access Services that are in service. The results are added to obtain the total count for a particular customer- designated premises and determines the applicable rate band. This step is repeated for each customer- designated premises.

When a facility carries both Switched and Special Access service under a Shared Use Arrangement as set forth in Section 6.8.17 following, each channel termination will be counted as either Switched Access or Special Access, but the total number of such circuits will be counted in determining the rate band charged.

(C) Service Description

With FMS, Feature Groups A, B, and D switched access services are provided to the customer over discrete channels. The Telephone Company does engineering of the service from the entrance facility of the customer’s primary premises to the end offices and/or tandem access switching offices over its Switched Access Network. The channel routing may not be designated by the customer as it is for most Telephone Company regular Switched Access High Capacity Services (see Section 5.2 preceding).

Facilities Management Service is an alternative to the customer's self-management of its network of standard switched access services, and will therefore be rated discretely (see Section 6.9.10 following).

(1) FMS is available in all LATAs, and is provided on a LATA basis.

(2) FMS is provided on a month-to-month basis or, at the option of the customer, under a three-year term plan or a five-year term plan. The minimum (D) period for FMS when provided on a month-to-month basis is one year. The minimum billing for individual channels within the FMS network is one month.

(3) FMS is available to any customer that meets the following minimum requirements:

(a) The customer must have at least one primary premises as defined in 6.8.26(B) preceding within the LATA.

(b) All of a customer's primary premises and the associated Feature Group A, B, and D services must be included in the FMS plan for that LATA.

(c) All Primary Premises in the plan must be of the same type (type 1 or 2).

N-1 carriers may arrange in advance to query, via the common channel signaling network, the Telephone Company's SPNP database which contains information necessary to route calls to number portable NXX codes. When the necessary routing information has been returned from the SPNP database to the switch originating the query, call processing is resumed by the originating N-1 carrier, and the call is routed to the correct network switching element for completion to the called party. The N-1 carrier will be assessed a SPNP Database Query Charge on all queries to the SPNP Database.

(D) SPNPDS Service Provisioning

(1) SPNPDS Provisioning

SPNP Database access is available in the Philadelphia and Washington LATAs. The database in Philadelphia LATA will provide LRN information on ported telephone numbers in the states of New Jersey, Pennsylvania, and Delaware. The database in the Washington LATA provides LRN information on ported telephone numbers in the states of Maryland, Virginia, and the District of Columbia. Customer (D) requests for SPNP Database Query Service will be treated as projects.

(2) Limitations

SPNP Database Service is to be used only on a call-by-call basis for routing calls to number portable NXX codes and cannot be used for purposes other than those functions described herein.

Information residing in the Telephone Company's SPNP database is protected from unauthorized access and may not be stored in a customer's database or elsewhere for any reason.

A customer ordering incidental InterLATA Service must, at a minimum, subscribe to a telephone company access service.

(A) Service Description

CCS - Gateway Access Service provides for the interLATA transport of incidental signaling information by the Telephone Company's SS7 network and routes these messages through a shared Gateway Access Port at the Signal Transfer Point (STP) hub.

The provision of CCS - Gateway Access on an interLATA basis by the Telephone Company is limited to SS7 signaling used in connection with the provision of services or exchange access services by a local exchange carrier and to common carriers offering interLATA services at any location within the area in which the Telephone Company provides telephone exchange services or exchange access service.

21. Contract Tariffs

This section contains terms and conditions and rates and charges for contract tariffs provided by the Telephone Company. Individual contract tariffs are filed in 21.2 following.

21.1 General

Contract tariffs apply in Metropolitan Statistical Areas (MSAs) which are eligible for Phase I and Phase II Pricing Flexibility. MSAs that are eligible for Phase II Pricing Flexibility are further subject to Level 1 and Level 2 MSA pricing. Telephone Company Phase II MSAs and Level 1 and 2 pricing eligibility within the operating territories of The Verizon Telephone Companies are set forth in this Tariff F.C.C. No. 1, Section 14.7; Tariff F.C.C. No. 11, Section 15.3; and Tariff F.C.C. No. 14, Section 19.1.

Each contract tariff includes a serving area that is comprised of one or more MSAs within the operating territories of one or more Verizon Operating Telephone Companies. This tariff provides for service within the operating companies of Verizon Pennsylvania Inc., Verizon Delaware LLC, Verizon Maryland Inc., Verizon Virginia Inc., Verizon Washington, D.C. Inc., and Verizon New Jersey Inc. whose operating territories are (D) specified in Sections 14.1 - 14.3 preceding. 25. Discount Plans

The following discount plans are available to all customers who purchase the applicable services and meet the requirements for each such discount plan, as set forth in Section 25.1 following for Commitment Discount Plans, 25.2 following for Service Discount Plans, and Section 25.3 following for National Discount Plan.

25.1 Commitment Discount Plans

A Commitment Discount Plan (CDP) provides for the application of a discount to the monthly recurring rates for service(s) included in the CDP based on a Minimum Commitment of channel terminations. For administrative purposes, all services included in CDP are managed as a single plan with separate commitment periods applicable to each service type. Customers may combine certain services together when establishing their commitment levels in accordance with Section 25.1.3 following. Only one (1) CDP is permitted per customer (i.e., one per legal entity).

25.1.1 Availability of a Commitment Discount Plan

(A) At the customer’s request, certain Telephone Company Switched Access Services and Special Access Services as set forth in (E) following are eligible for inclusion in CDP. The types of services included in the CDP are set forth following.

(B) CDPs for Switched Access Service Direct Trunked Transport are only available in states where Collocated Interconnection as set forth in Section 19 preceding has become operational and either:

(1) a total within the state of 100 DS1 equivalent Entrance Facility Channel Terminations have been provided in the Zone 1 serving wire centers, access tandems and remote nodes; or

(2) an average of 25 DS1 equivalent Entrance Facility Channel Terminations have been provided per Zone 1 serving wire center, access tandem or remote node in that state.

25.3.6 Special Access DS3 Service Channel Mileage Rates

(A) Special Access DS3 Service Channel Mileage Banded Base Rates set forth in Section 7.5.9(B)(1)(e)(ii) preceding will apply to Special Access DS3 Services that are provided between two serving wire centers of the same Verizon Telephone Company, and to the following configurations in which Special Access DS3 Service is jointly provided by more than one Verizon Telephone Company under the provisions of Section 2.4.7 preceding.

- Special Access DS3 Services provided jointly by Verizon Pennsylvania Inc. and Verizon Delaware LLC in LATA 228.

(B) Except as set forth in (A) preceding, the Channel Mileage Base Rates set forth in Section 7.5.9(B)(1)(e)(i) preceding apply to all other Special Access DS3 Services that are included in the NDP and provided between (i) two (2)Verizon Telephone Companies; or (ii) between a Verizon Telephone Company and another exchange telephone company in accordance with Section 2.4.7 preceding. For example, the Base Rates as set forth in Section 7.5.9(B)(1)(e)(i) preceding would apply to the Verizon New Jersey Inc. portion of a Special Access DS3 Service provided between a point in the North Jersey LATA and a point in the New York Metropolitan LATA of the New York and New Jersey Corridor.
