22 The Ryan White Story

You see the picture of one of the greatest people to grace this earth. His name is Ryan White and he died with AIDS. His story is as follows:

On December 6, 1971, Ryan Wayne White was born. Three days later, doctors told his parents that he was a hemophiliac. That means his blood does not clot the way it should. Luckily, there was a new product out that contained the clotting agents found in blood. This product was called Factorate, and was made from blood. Ryan grew up having many hemorrhages, or bleeds, and would get infusions of Factorate twice a week.

While he was battling pneumonia, he had to have surgery on December 17, 1984, to have two inches of his left lung removed. Two hours after the surgery, doctors told his mother that he had contracted AIDS. He was given 6 months to live. But he was a fighter.

He was determined to continue at his school and live life normally. But in 1985, not many people knew the truth about AIDS. Not very much was known about AIDS. Ryan faced a lot of discrimination, mostly based on the unknown. His school tried to keep him from attending and the town in which he lived was not very supportive, to say the least.

After legal battles, Ryan and his mother settled with the school to have separate restrooms and disposable silverware from the cafeteria. But that didn’t stop much. Students vandalized his locker with the word “FAG” and restaurants threw his dishes away after he left. A bullet was even fired into his home.

After that, he and his family moved to Cicero, Indiana, and were welcomed. People had been educated and Ryan was happy again. He had his learner’s permit. He had friends, his family, and he was turning an incurable disease into a way to educate America. There was a movie made about him, titled “The Ryan White Story,” that aired on ABC. Ryan got to be an actor as he played his friend Chad.

Ryan White was making a difference. But on April 8, 1990, the world lost a wonderful person. He knew he was famous, took that, and turned it into something that everyone could learn from. He knew his purpose in life.

23 Ryan White HIV/AIDS Treatment Extension Act (formerly known as Ryan White CARE Act)

The Ryan White HIV/AIDS Treatment Extension Act of 2009 provides the Federal HIV/AIDS programs in the Public Health Service (PHS) Act under Title XXVI flexibility to respond effectively to the changing epidemic. The new law changed how Ryan White funds can be used, with an emphasis on providing life-saving and life-extending services for people living with HIV/AIDS across this country. The Ryan White HIV/AIDS Program was enacted in 1990 and, in addition to 2009, was reauthorized in 1996, 2000 and 2006. The major parts of the Ryan White Program are: ● Part A (Title I): Funds “eligible metropolitan areas” (EMAs), those with cumulative total of more than 2,000 reported AIDS cases over most recent 5-year period, and “transitional grant areas” (TGAs), those with 1,000–1,999 reported AIDS cases over most recent 5-year period. At least 75% of Part A funds must be spent on core medical services. ● Part B (Title II): Funds all 50 States, the District of Columbia, Puerto Rico, Guam, the U.S. Virgin Islands, and 5 other territories and associated jurisdictions. Includes Part B base and supplemental grants, ADAP and ADAP supplemental grants, and Emerging Communities (ECs) grants. At least 75% of funds must be spent on core medical services. Funds are distributed by formula to states based on a state’s share of living HIV and AIDS cases, weighted to reflect the presence or absence of EMAs/TGAs. • ADAP & ADAP Supplemental: Funds are “earmarked” by Congress for state ADAPs to provide medications to people with HIV/AIDS (or pay for health insurance that provides medications). • ECs: A portion of Part B base funds are set-aside for grants to metropolitan areas that do not yet qualify as EMAs or TGAs, but have 500-999 cumulative reported AIDS cases over most recent 5 years. All funding is distributed via formula.

24 ● Part C (Title III): 75% of funds must be spent on core medical services. Public and private organizations are funded directly for: • Early Intervention Services (EIS): to reach people newly diagnosed with HIV. Services include HIV testing, case management, and risk reduction counseling. • Capacity Development & Planning Grants: supports organizations in planning for service delivery and in building capacity to provide services. ● Part D (Title IV): Funds public and private organizations directly to provide family- centered and community-based services to children, youth, and women living with HIV and their families. Services include outreach, prevention, primary and specialty medical care, and psychosocial services; Part D also supports activities to improve access to clinical trials and research for these populations. ● Part F: Includes: • AIDS Education and Training Centers (AETCs): national and regional centers that provide education and training for health care providers who treat people with HIV/AIDS; and dental reimbursement and community-based dental partnership programs. • Minority AIDS Initiative (MAI): The MAI, created in 1998 in response to growing concern about the impact of HIV/AIDS on racial and ethnic minorities in the United States, provides funding across several DHHS agencies/programs, including Ryan White, to strengthen organizational capacity and expand HIV-related services in minority communities. • Special Projects of National Significance (SPNS): address emerging needs of clients and assist in developing standard electronic client information data system. SPNS is funded through “set-asides” of general Public Health Service evaluation funding, separately from the amount appropriated by Congress for Ryan White.

The Ryan White HIV/AIDS Program requires the Atlanta Eligible Metropolitan Area to appoint a Planning Council. Planning Council members include community representatives, consumers of services, community based organizations, and service providers. The Planning Council responsibilities include identifying the needs of Persons Living with HlV/AIDS, preparing a comprehensive plan, setting priorities for the allocation of funding, and evaluating how well services meet community needs. I found out I'm positive - What should I do? • Find a doctor who will work with your insurance • You are encouraged to look for an Infectious Disease Specialist • If you don't have insurance, there are many programs to help you pay for health care and medications • Try to get a doctor you feel comfortable with • Even though you feel good, going to the doctor for preventative care is important.

What Else Can I Do? • Get educated about HIV • Get support - AIDS Service organizations (AS0s) - Family/friends • Get rid of bad habits - Stop smoking - Stop recreational drugs and excessive alcohol intake - Eat well - Exercise

What does HIV do to my body? • Viral load = how much HIV is in your blood • CD4 cell = type of blood cell that fight. infections; also called T-cells - HIV damages CD4 cells - With fewer CD4 cells, your body is less able to fight infections.

What is the difference between HIV and AIDS? • HIV is a virus that infects cells in your body • You may be diagnosed with AIDS when your CD4 cell count declines below 200 and/or you are diagnosed with infections known as "opportunistic infections " • Everyone with AIDS is infected with the HIV virus, but not everyone with HIV has AIDS • The lower your CD4 cell count, the greater your risk for infections.

Can HIV be treated? Yes, HIV can be treated! • Talk to your doctor about when to start therapy and what medicines are right for you • There are many different medicines that fight HIV infection

28 • Not everyone with HIV needs to start treatment right away - The amount of virus in your blood (viral load) and the amount of damage to your immune system (CD4 cell count) are important in making this decision.

What else do I need to know about starting treatment for HIV? • Medicines work best if you take them every day • Be sure to follow your doctor's directions about when to take your medicines and whether to take them with food or water • If you don't follow your doctor's directions, the medicines will not work as well

How will I know if the medicines are working? • Even if you feel fine, you should see your doctor at least every 3 months • During your visit, the doctor will draw blood to check the amount of virus in your blood (viral load) and the damage to your immune system (CD4 cell count) • When the medicines are working, your viral load will go down and your CD4 cell count will go up

What about side effects? • All medicines that fight HIV can cause side effects • Talk to your doctor about what side effects can happen with your medicines • If you are having trouble taking your' medicines because of side effects, don't stop taking them without talking to your doctor first. Your doctor may be able to help.

Summary • Anyone can get HIV • The only way to know if you have HIV is to get tested • HIV is treatable • Many resources are available for people with HIV and those who care for them

32 Proof of Residency You may use one of the following to show proof of residency: 1. Lease or deed with your name on it. This can also be a sublet agreement. 2. Utility Bill; this must be in your name. This cannot be a cell phone bill and cannot be a Social Security form. It must be a gas, electric, or landline phone bill with the address and your name on it. 3. Letter of Homelessness: ● A shelter letter: This letter must be recent and state that you had slept at that shelter. This assists in identifying county of residence. ● The Letter of Homelessness typically is completed by a case manager, stating that you have been homeless for a specific period of time. This is intended for individuals who are sleeping on the street or squatting in an abandoned building, etc. A notarized letter is preferred. 4. Letter from landlord/roommate, etc. If you are living with someone, but do not hold a lease in your name, the person who does hold the lease can write a letter on your behalf. This letter should: ● State the address where they reside ● Have a signature (not printed) of the lease holder ● Rent paid by client ● State whether they share other resources* ● Be notarized.

If they do share resources, documentation is needed of that person’s income and Social Security number. It will be counted as the client’s income and may affect eligibility.

The Georgia Department of Public Safety began fingerprinting in September 1996 so if you have gotten your ID or Driver’s License
