No man, woman or child should go blind just because they cannot afford the treatment.

LRBT ANNUAL REPORT 2012 The Layton Rahmatulla Benevolent Trust Of your five senses, which one are you most afraid of losing? If you are like most people, the answer is the ability to see. Sight is precious.

Like other parts of the body, the eyes start to undergo changes as they age. Our eyes change as we get older. The cornea becomes more opaque, the lens absorbs more light, and the pupil diameter decreases.

Eyesight deteriorates with age. As we age, natural physical changes of the human eye diminish our visual abilities. Vision worsens with time. In fact, vision problems are one of the first signs of aging. Many people have to start wearing glasses between the ages 5of 40 and 50 because of failing eyesight. Some of the disorders that pose a threat to vision as we grow older are: l Cataracts Age is the main risk factor for these l Glaucoma diseases. For example, l Age-related Macular half of the people between the ages of Degeneration (ARMD) 65 and 74 have l Diabetic Retinopathy cataracts, after age l Dry Eye Syndrome 75, about 70 percent do. And glaucoma is most common in people over 60.

The elderly population of Pakistan is increasing rapidly. Currently about 12 million people, or 6.6 percent of the total Loss of vision among the elderly population, are over is a major health care problem as the age of 60; about half of the 12 million by 2050, this number is people above the age of 60 in expected to swell to Pakistan are either blind or 14.3 million people, or visually impaired. almost 16 percent of the country’s expected 5population.

The effects of the breakup of the traditional family structure and support system are heart rending for the elderly in both the urban areas and rural communities of 01Pakistan. Noor Muhammad, a 76 year old farmer from Balochistan, has spent many years “God bless LRBT and living in a mud hut alone. He lost his wife ten years ago and his children had the kind donors who all moved away, contacting him infrequently. have given me the gift of With age Noor’s health had deteriorated and he was dependent on his neighbors and friends for food, sight at a time when company and care. Although he did not enjoy being a burden upon them, his I was surrounded with unfailing optimism, charming demeanor and his age old friendships darkness. They have helped him through his troubles. When Noor’s eyesight began to fail, he was unable to find work. As he had restored my dignity and no income, he went for no medical treatment. Finally he could not see at my faith. I can look after all. Confined to his hut and entirely dependent on his neighbors and friends for even the simplest of tasks, myself now and look Noor sank into a deep depression. The constant darkness he was confined to forward to the future.” made him think of ending his agony by killing himself. Sometime later one of Noor’s daughters came to visit him and was thoroughly shaken by her father’s condition. By chance she had heard of LRBT’s operations and hospital in Quetta and Noor arranged to take him there. At LRBT Noor was diagnosed with cataracts in both his eyes and was Muhammad immediately given a date for his surgery. Following a successful procedure and a very positive post operative examination Noor was thrilled that he was able to see again. With immense gratitude and delight he said “When I lost my sight I was extremely worried. With my wife’s death and my son’s departure I did not know how I would survive or who would continue to take care of me.

Vision impairment – the dimming of sight then blindness – inevitably leads to unemployment and loss of income. Zohra is a 65 year old widow from 02Karachi. Ostracized by her community and blamed for her husband’s death, she was left alone to take care of her 5 “LRBT has not only given children. She and her children slept on the floor of a single room while she me back my sight but took up tailoring, working day and night to support her children and put them through school. has given me back hope Zohra Bibi had the misfortune of having a sharp object injure her right for the future and has eye. Distraught by this news and unable to afford the cost of surgery, she continued to work and support her l family. When she could no longer work, her children had to drop out from college. Fortunately, a family friend told Zohra about LRBT’s free eye hospital. Following a successful procedure at LRBT, she could see clearly again. She says: “All my dreams were shattered when my children had to give Zohra up their studies to support us. Now that I have regained my sight, I will go back to stitching so that I can support them Bibi and they can continue with their education.

Blindness leads not only to social isolation but to loss of daily income and to poverty. Abdul Sattar a 60 year old office helper from Multan was the sole breadwinner for his family of six. Despite earning merely 03Rs. 7,000 a month he lead his life with dignity. “I could never Unfortunately Abdul Sattar’s vision began to fail and knowing that he could not afford the treatment he shied away from imagine that I would seeking help. Worried and distraught he visited the local ophthalmologist who diagnosed cataracts in both his eyes and asked for LRBT has restored my Rs. 30,000 for the procedure. Abdul Sattar found himself faced with a dignity and has saved decision to make. He could either have the surgery and his family would surely me from an old age starve, or he could continue as he had been but was sure he would lose his job and his family would starve in the future. full of darkness.” Either option was terribly bleak. He chose i the latter and soon Abdul Sattar’s vision deteriorated to the point where he could only see shadows of the faces around him and was asked to leave his job. Confined to a corner of his home, he was entirely dependent on his family for help with even the simplest of tasks. Abdul Sattar found himself bumping into things and breaking fragile items, ruining food stuff and generally upsetting the household. His wife, who lovingly cared Abdul for him for many years, began to nag him incessantly; his children soon became Sattar intolerant of him; his brothers, who now had to look after him and his family, taunted him and made sure he felt worthless. A once dignified and proud man was reduced to a pitiable wreck who found himself praying for either a miracle or death. Abdul Sattar fortunately received his miracle. An old colleague came to visit him and was heartbroken to see his state. Having heard about LRBT’s free eye hospital, he took Abdul Sattar there immediately, where he was diagnosed with hyper mature cataracts and underwent two successful procedures. Filled with relief and happiness, he ecstatically proclaimed “It’s like I am dreaming,

In addition it has 39 students in certificate programmes for Ophthalmic Technicians, Refractionists and Operating Theatre Technicians in its School of Ophthalmic Paramedicine at Korangi hospital. Chairman’s Report

The growth of patient visits to our hospitals and Community Eye Health Centres (CEHC) continued unabated. OPD was higher by 7 % and major surgeries by 12 %. We remain the largest provider of eye care in the country treating 36 % of all eye patients visiting hospital, performing 27 % of eye surgeries and 32 % of paediatric cataract surgeries. All treatment at our 17 hospitals at 44 CEHCs is free as we focus on those who live around the poverty line and can barely feed their family.

Double digit inflation in the cost of medicines, surgical supplies and salaries of doctors and paramedics along with high patient volume growth is straining our finances. Despite our lean manning, high productivity and efficient procurement our operating expenses were higher than our income from grants, donations and zakat. This is a cause of concern as inflation and patient volume growth is unlikely to abate in future years. We are working to enlarge our donor base both within and outside the country to ensure that we can continue to serve as the only national safety net that the deprived and cunderprivileged people of our country have for quality eye care. Despite our constrained finances we have not stopped the expansion of those hospitals √ Lar and Khanewal - which required extra capacity to meet increasing patient volume or the 2 new hospitals one at New Karachi which opened in January 2012 and Odigram (Swat) which will be completed by the second half of 2013. I should add that on an average over the last 27 years we have opened a new hospital every 18 months; this besides the expansion of our existing hospitals.

I would like to thank the Sindh Government»s Community Development Programme for continuing to fund cataract surgeries at 4 of our hospitals among the poor people of the province. Although the number of surgeries was reduced to 12,381 from 18,750 in the previous period it will still help in alleviating poverty and generating increased family income by converting the patients into income generating individuals. I would also like to express our gratitude to USAID»s Small Grants Ambassador»s Fund Programme for funding 11,417 cataract surgeries amongst female patients at 7 of our hospitals in all 4 provinces. This will help to reduce female blindness, empower women and increase family income.

LRBT LRBT Advisory Council consisting of 24 eminent personalities and social workers was established to advise,

Annual Review 2012 Review Annual guide and assist the Board and the organization in carrying out its mission.

I would like to thank the media for once again giving us free time/space for our Zakat campaign which helped in making it a great success. Our staff has worked hard in difficult situations and at times under physical danger and I would like to thank them for their services which were well above the call of duty.

Finally our donors who are our life blood and whose continued and enhanced support over the last 27 years have made possible the significant impact made by us in the prevention of blindness and impaired vision .The 22 million people who have benefited from our services owe them a deep debt of 16 gratitude. May God bless them all and reward them adequately in this world and the next. LRBT Annual Review 2012 17 Review of Operations

OPD VISITS / SURGERIES During the year ending June 2012, 2.2 million patients visited our 17 hospitals and 44 Community Eye Health Centres, a growth of 7% over the previous year. During the same period 221,760 major & minor surgeries were performed. Growth in major surgeries was 12 % over last year. Cataract which is the leading cause of curable blindness constituted 92 % of major surgeries but only 69 % of total surgeries. Other surgeries included Vitreo Retina, Trabulectomy (Glaucoma), Cornea transplants, DCR, Squint etc. We remain the biggest provider of Vitreo-retinal surgery and Corneal transplants for the poor people in the country.

190 corneas were transplanted at our Lahore, Korangi and Quetta hospitals: 108 in Lahore with corneas being donated by Begum Zahra Manzoor Illahi, President Binai Eye rBank Pakistan, 81 in Korangi and 1 in Quetta. Corneas were donated by the Toronto Eye Bank, Canada courtesy Syed Muslim Hasany, Pakistan Eye Bank Society and Mr. Jamal from Karachi.

3,865 patients were treated with Avastin for Age Related Macular Degeneration

EXPENSES Our operating expenses during the year were Rs. 535 million being 13.4% above last year, mainly due to setting up of a new hospital in North Karachi, a growth of 7% in patient visits to OPD, 12% in major surgeries and higher expenditure on Fundraising activities & Human Resource training. Capital Expenditure at Rs. 89 million (5% higher than last year) included donated land of Rs. 29.6 million for construction of hospital building in Odigram, Swat and donated property of Rs. 11.4 million for setting up of new hospital in North Karachi. Our overhead expenses at 4.45% of operating expenses are amongst the lowest in the world.

EXPANSION OF CAPACITY

LRBT LRBT KhANEwAl

Annual Review 2012 Review Annual Work on the new Complex is progressing according to schedule and is expected to be ready by the end of 2012. With a significant increase in space being made available for the convenience of patients, there will be a smooth flow and movement of patients visiting the hospital for treatment. The complex will provide new consultation rooms, biometry, laboratory, dispensary, laser treatment room, refraction room and an additional major and a minor Operation Theatre. We are very grateful to our generous donor in the UAE who is funding this entire venture at an estimated cost of Rs.14.42 million. This donor is also graciously donating the required equipment at a cost of Rs.17.76 million spread over three years. 18 AKORA KhATTAK The hospital was reopened following its closure for nearly three months due to security concerns after kidnapping of our hospital administrator from the premises. Meanwhile, a new and larger Operation Theatre was constructed which is now operational. Surgical capacity has increased with the addition of 3 operating tables. lAR (MUlTAN) With a steady increase in the inflow of patients requiring surgery and limited capacity of the operation theatre, the backlog had built up to a stage where patients had to wait for many months before their surgery could be carried out. A new and expanded operation theatre was, therefore, planned to be made which would go a long way in resolving this issue. The larger theatre has a provision for 8 operating tables. Scheduled date of completion was July 2012 but the project was completed and commissioned on 18th June. There is also dearth of suitable accommodation in the vicinity of the Hospital and in its absence doctors have to commute from long distances which is difficult in the absence of a reliable transport system. In order to address this problem, one family and two unmarried doctors, residences have been built and the project completed in October 2011.

PASRUR: In order to meet the shortfall of accommodation within the hospital premises, a residence for a married doctor and two residences for unmarried doctors have been constructed so that doctors are readily available at the Hospital to provide treatment to patients. This project was completed in August 2011.

ARIFwAlA: Presently, visiting surgeons face a lot of difficulty during their visits due to paucity of accommodation at the Hospital and its vicinity. In order to also make Arifwala an attractive place for those surgeons posted there, accommodation for one family and two unmarried doctors is under construction and is expected to be ready by the end of December this year.

KORANGI In order to provide comfort to patients a new block of eight toilets has been constructed and a new sewerage system has also being laid. LRBT LRBT

Construction work on the first floor of the staff residential block and a mess/kitchen on the 2012 Review Annual ground floor was initiated to provide accommodation for trainees undergoing the Refractionist and OT Technician courses at our School of Ophthalmic Paramedicine .
