Ansari, [email protected] Abstract A fecalith is a common cause of acute appendicitis, and laparoscopic surgery is the mainstay of its management. Literature review shows that a fecalith may be retained in the gut following a laparoscopic appendectomy in some rare cases. In most cases, the fecalith becomes symptomatic with time due to the formation of an abscess, fistulous tract, or inflammation of the appendicular stump (stump appendicitis). We report a case of retained appendicular fecalith presenting with symptoms similar to acute appendicitis, 15 years after laparoscopic appendectomy. Fecal impaction occurs when a large amount of fecal matter gets compacted and cannot get evacuated spontaneously [1]. In its extreme form, fecal impaction can lead to the formation of a fecalith due to the hardening of fecal material that forms a mass separate from other bowel contents [2]. It can occur in any part of the intestine [1]. Most often, a fecalith arises in the colon (mostly sigmoid) or rectum and very rarely in the small intestine [2]. Here we present a case of a retained appendicular fecalith in a patient who presented with an acute abdomen. -

This laxity of attachments may even cause torsion of the splenic pedicle. Both gastric volvulus and wandering spleen share a common embryology owing to mal development of the dorsal mesentery. Gastric volvulus complicating a wandering spleen is, however, an extremely unusual association, with a few cases described in literature. Case Report We report a case of a young female who presented with acute abdominal pain and vomiting. Radiological imaging revealed an intrathoracic gastric. Conclusionvolvulus, torsion in an ectopic spleen, and additionally demonstrated a pancreatic volvulus - an unusual triad, reported only once, causing an acute abdomen. The diagnosis can prove clinically challenging, and hence there is increased reliance on imaging. There are different types of gastric volvulus, with the variant presented in our case being the less commonly encountered mesenteroaxial gastric volvulus. Some of the CT features of gastric volvulus are described, and the usefulness of CT in assisting with the diagnosis is highlighted. S Afr J Rad 2013;17(1):21-23. DOI:10.7196/SAJR.817 Gastric volvulus is a rare clinical entity, and a clinically relevant cause of acute abdominal pain in adults. It may prove to be a diagnostic dilemma for clinicians in view of the nonspecific clinical symptoms. The imaging diagnosis also remains challenging.[1] Abdominal computed tomography (CT) has been underutilised in a b c the diagnosis of gastric volvulus in previously reported series.[2] The present case illustrates the value and importance of abdominal CT in the rapid diagnosis of acute abdominal pathology and, in particular, acute gastric volvulus. CASE REPORT An 87-year-old female patient, with previous history of high blood pressure, came to the Emergency Department with complaints of intensive abdominal pain in the last 24 hours, associated with nausea without vomiting. Physical examination determined blood pressure: 120/60 mmHg, heart rate: 110/min, none diuresis, abdominal distension and epigastric pain without signs of peritonitis. Lab report: 12.900 leukocytes/µL (90 % neutrophils), creatinine level: 3 mg/dL, urea level: 90 mg/dL. A nasogastric tube was inserted and abundant gas was obtained through it. Thorax and abdomen radiography scan were made (Figs. 1 and 2). A CT-scan was also performed (Fig. 3). This patient underwent an emergency laparotomy and a complete gastric necrosis was found (Fig. 4). A total gastrectomy was initiated but the patient suffered an intraoperatory cardiac arrest.
