Although at first his principles were hardly accepted by his colleagues, his work was confirmed with many studies worldwide and his technique became the basis of modern practices. Ilizarov external fixator evolved from being a last resort solution to a method used to treat a great number of bone disorders. One of the most famous principle is “distraction osteogenesis” also called “callotasis” or “callus distraction”, a technique involving slow distraction of the callus in response to a corticotomy. It has the benefit of increasing both bone length and surrounding tissues. The purpose of this article is to present an overview of the history of Ilizarov apparatus, the mechanical laws that underlie it, the current clinical use, the most important complications and future research. Pelvic trauma carries a significant morbidity and mortality. The development of pelvic binders and their utilization has reduced this risk by reducing intra- pelvic volume, encourage clot formation and reducing blood loss. However, are these devices being used effectively and is sufficient investigation being performed? Methods: A retrospective review of electronic records from November 2018 – February 2019 was conducted within the major trauma centre, with two independent reviewers. Exclusion criteria included: insufficient imaging and paediatric trauma. Analysis of binder position and post-removal imaging was performed. Results: 177 cases were included for review. Analysis revealed 44% of cases demonstrated inappropriate positioning of pelvic binder. 76% were sited above the optimum level of the mid-trochanteric point. There was a mean distance of 58.8mm from the optimum position, with a range from 22m to 128mm. Of these patients 9% had appropriate complete trauma imaging in the form of whole body trauma CT scanogram, with 3.2% of those without a pelvic fracture undergoing a post-binder removal plain pelvic radiograph. Conclusion: Binder position is essential in effective haemorrhage control. This study demonstrates within the major trauma setter only half are positioned optimally.

    COMMENTARY Surgical innovation in the cold war era: Gavriil Ilizarov and his apparatus as a device for external fixation and limb lengthening Boluwaji Ogunyemi1 Citation: UBCMJ. 2017: 8.2 (29-30) Abstract Gavriil Abramovich Ilizarov [1921-1992] was a researcher and innovator of orthopedic surgery. In the setting of the Second World War, he was forcibly evacuated from Crimea where he studied medicine. He finished his studies in Kazakhstan before working in rural Siberia. Being positioned behind the Iron Curtain complicated the dissemination of the surgeon’s apparatus to North America. From rudimentary materials in rural Siberia, Ilizarov pioneered an external fixator used to lengthen limbs and treat comminuted fractures. Introduction 1.1 Distraction osteogenesis Bone is amongst the very few tissues in the human body that possess intrinsic capacity to heal spontaneously following injury. However, beyond a certain critical size defect, bone cannot heal by itself and outside intervention is required. Numerous techniques are available for the management of these defects, including the gold standard autogenous bone grafts, allografts, bone graft substitutes, vascularized fibular bone grafts and systemic administration of anabolic agents. All these techniques, however, do have limitations (Dimitriou et al., 2011; Nauth et al., 2011). Such instances of severe bone loss, whether due to congenital bony deficiencies or acquired causes, pose an immense challenge to the treating physicians, and it is in these cases that distraction osteogenesis could offer a viable and successful alternative to these techniques. Distraction osteogenesis (DO) is a surgical technique in which the intrinsic capacity of bone to regenerate is being harnessed to lengthen bones or to replace large segments of bone. It consists of the application of an external fixator to the affected bone (Figure 1), followed by an osteotomy of the bone and then gradual and controlled distraction is applied to the two bone segments. This controlled distraction, usually by an external fixator, generates new bone within the distracted gap. When the desired lengthening is obtained, distraction is stopped and the external fixator is kept on until the newly formed in the distracted gap is mechanically strong enough to allow removal of the fixator.

Focusing on material objects ranging from space rockets to heritage buildings to weightlifting equipment to TV sets, it argues that material objects in late socialism were key elements in the organization of the Soviet historical and spatial imagination. They embodied various, often contrasting social techniques and understandings of time and space, and acted as material coordinates of the Soviet self. The central concept of this dissertation is elemental materialism, by which I mean a culturally rooted recognition of the power of matter and things to shape human bodies and selves, a prominent feature in the Soviet system of signification which regulated the production of meanings on daily basis. Soviet elemental materialism was a social reaction to pre-ideological experiences of daily life, including entangled assemblages of bodies, objects and physical space which exercised a social agency that did not originate from the dominant Soviet ideology. It was a set of spontaneous and situational cultural forms which gave Soviet people ways of making sense of the social agency of things. At the same time, my research historicizes Soviet things and material space in their spontaneity and affectivity as actual agents of historical change in the late USSR on a par with people, social institutions, and ideologies.
